Orthodontic Evaluation and Craniofacial Characteristics of a Patient with Dubowitz Syndrome: A Case Report

This case report presents the orthodontic evaluation and treatment planning of a patient with Dubowitz syndrome, an autosomal recessive inherited and rare genetic disorder characterized by microcephaly, growth retardation, high sloping forehead, facial asymmetry, micrognathia, deep palate, blepharophimosis, sparse hair and eyebrows, low ear and mental retardation. The male patient, with a chronological age of 8 years and 2 days, was referred to our clinic due to severe overjet and mandibular retrognathia. Radiographic and clinical evaluations revealed deep palate, narrowness of maxillary and mandibular arches, mandibular micrognathia, speech difficulties and mixed breathing. The patient's treatment planning was aimed at function and phonation and included the use of a twin block appliance with expansion screw in both parts and simultaneous use of occipital headgear (350 g force on one side). It was aimed to positively direct the growth in the craniofacial structure by making use of the continuation of the patient's growth and development period, and to prevent interventions such as surgical-assisted rapid maxillary expansion and orthognathic surgery, which are invasive approaches in adulthood.
Comparison of the Effect of the Time Under the Three Primary Color Lighting of Led Production Before Scanning of Phosphorus Plates

Purpose
This study aims to compare the effect of light exposure in red, green and blue (RGB) colors prior to scanning of the PSP plates.
Materials & Methods
An Arduino-based system is produced for standardized light exposure to the irradiated PSP plates. The system consisted of an Arduino Mega 2560 developer board, 2 RGB LED light sources, a TSL2591 digital light sensor and a DHT11 temperature and humidity sensor. A light-tight platform is produced with additive manufacturing and electronical units are integrated into this platform. A two-step alloy was used to create contrast. PSP system (VistaScan, Dürr Dental, Germany) is irradiated with fixed parameters of 70 kV, 8 mA and 0.5 seconds. Scanning of the PSPs were delayed for 1-,3-,5-, and 10-minutes, and half of the active surfaces are exposed to RGB lights independently in full brightness (PWM) and calibrated with lux while the rest is protected. MGVs are measured in 6 regions per image. The MGV differences in regions between conditions were examined by Kruskal-Wallis test. A p-value<0.05 was considered statistically significant.
Results
In PWM setup, signal loss was higher in blue color at 1-,3-, and 5-minutes, but at 10-minutes, only the green light produced half-image. In LUX setup, signal loss was lower in green light. Contrast loss was lower in green light with LUX calibration. (p<0.05)
Conclusion
Among three colors compared, effect of exposure to green light prior to scanning of the irradiated PSP plates is found to be lower than the other colors
Prevalence and Complications of Mesiodens: a Radiographic Study in a Group of Turkish Pediatric Population

Objective: This study aims to assess the prevalence of mesiodens in a Turkish pediatric population and to radiographically investigate the characteristics of mesiodentes and the complications caused by them.
Materials and Method: This study examined panoramic radiographs of 8002 patients aged 4-16 years. Patients were evaluated for age, gender, dentition status, the number, shape, and position of mesiodens, and complications it causes.
Results: A total of 39 mesiodentes were found in 30 patients. the gender ratio was 2:1, but the difference was not statistically significant (p=0.2). Mesiodentes were most common in the conical form, followed by the tuberculate and the least supplemental. Seventy-seven percent of the mesiodentes are located vertically, 15% horizontally, and 8% inverted; 51% erupted, and 49% impacted. With a rate of 43%, displacement or axial rotation of the permanent central incisors was the most common complication. The delayed eruption was the second most common complication (%37). Diastema was noted in 27%. Cyst formation was noted in 7%. There was a statistically significant relationship between the number of mesiodentes seen simultaneously and the number of complications (p=0.022).
Conclusion: The prevalence of mesiodens was determined to be 0.37% in the analyzed population, consistent with the literature. At least one mesiodens-related complication was noted in 80% of patients. As for complications, the number of concurrent mesiodentes observed is significant. Complications can be avoided by early diagnosis and timely surgical intervention.
Evaluation of the Relationship Between Gingival Pigmentation and Smoking

The abnormal deposition of melanin in the oral mucosa results in melanin pigmentation. The gingiva that has undergone keratinization is most affected. Gingival discoloration may be caused by physiologic or pathological reasons. Melanocytic lesions or melanocytic pigmentations induced by an excessive deposition of melanin in the basal and suprabasal layers of the epithelium are the most common pigmentations of the gingiva. Smoking has been associated to oral melanin pigmentation, particularly in the anterior gingiva. The Fagerstrom Test for Nicotine Dependence is the most used instrument for assessing smoking addiction (FTND). The Fagerstrom Test for Nicotine Dependence (FTND) consists of six questions, with a distinct score assigned to each. Our study comprised 255 individuals over the age of 18 who were smokers, did not use drugs, did not have any systemic illnesses that could contribute to pigmentation, and did not have any active infectious infections. The Hedin scale was used to assess the pigmentation levels of the subjects. The Fagerstrom Test for Nicotine Dependence was used to diagnose the kind of cigarette addiction exhibited by the subjects. Using the Fagerstrom questionnaire, we observed that pigmentation was more common as smoking tendency inreased.
Comparison of the Dental Treatments Performed in A Pediatric Dentistry Clinic During the Covid-19 Pandemic Period with the Pre-Pandemic Period: A Retrospective Study

Purpose: This study aimed to assess the impact of COVID-19 pandemic on attendance and treatments performed in a paediatric dental clinic.
Materials & Methods: A retrospective study was conducted using the records of pediatric patients treated in two periods: before the pandemic (15 March 2019–14 March 2020) and during the pandemic (15 March 2020–14 March 2021). Data regarding patients’ age, sex and administered treatment were collected and analyzed. The treatments applied were categorized under 6 groups: examination, restorative, preventive, prosthetic, emergency and surgical, and periodontal. Rates of each procedure were compared between two periods.
Results: During pandemic, 11,700 patients applied to our clinics and 26,995 procedures were performed, resulting a decrease by 63.5% and 84.3% in the outpatients and treatments, respectively, compared to the pre-pandemic period. During the pandemic, the rates of examination and surgical procedures were significantly higher than those before the pandemic, while the rates of other procedures were significantly decreased (p<0.001).
Conclusion: The COVID-19 Pandemic has been shown to have a significant impact on dental care delivery in children.
