dc.description.abstractSorafenibin Hepatoselüler Karsinomada PD-L1 Üzerine Etkisi Hepatoselüler karsinoma kansere bağlı ölümlerde başta gelen nedenlerden biridir. HCC'nin tedavisinde en sık kullanılan sistemik ilaç sorafenibtir. Sorafenib, hasta sağkalımını yalnızca birkaç ay uzatabilmekte ve zamanla gelişen direnç sebebi ile etkisiz hale gelmektedir. HCC'de artmış PD-L1 düzeyleri kötü prognozla ilişkilendirilmiştir. Bu çalışmada sorafenibin, immün kontrol noktalarında önemli bir yeri olan PD-L1 üzerine etkisinin araştırılması amaçlanmıştır. Çalışmada 2 farklı insan hepatoselüler karsinoma hücre hattı (HEPG2 ve HUH7) kullanılmıştır. Hücrelere 1, 5, 10 ve 20 μM olmak üzere farklı dozlarda sorafenib, 24 saat ve 72 saat boyunca uygulanmıştır. İnkübasyon sonrasında toplanan hücrelerde eş zamanlı PZR yöntemiyle PD-L1 düzeylerine bakılmıştır. Ayrıca 72 saat inkübasyon sonrasında, yüzeylerinde PD-L1 eksprese eden hücre oranları akım-sitometri ile tayin edilmiştir. Eş zamanlı PZR sonuçlarına göre; HEPG2 hücrelerinde 24 saat sonrasında kontrole göre sorafenibin tüm dozlarında PD-L1 ekspresyonunda azalma görülürken; 72 saat sonrasında yalnız 20μM dozda kontrole göre istatiksel olarak anlamlı bir artış izlenmiştir. HUH7 hücrelerinde de benzer olarak 1-10μM dozlarında24 ve 72 saat sonrasında kontrole göre anlamlı olmamakla birlikte bir azalma izlenmiştir. Benzer olarak akım-sitometrisinde de PD-L1 eksprese eden hücre oranları, HEPG2 hücrelerinde sorafenibin düşük dozlarında (1 ve 5 μM) kontrole göre azalırken; yüksek dozlarda artış görülmüştür. HUH7 hücrelerinde ise sorafenibin 1 μM dozunda belirgin bir fark izlenmezken diğer dozlarında PD-L1 ifadesinde belirgin bir artış izlenmiştir. Bu sonuçlar, sorafenibin düşük dozlarında ve kısa süreli uygulanmasında PD-L1 ifadesini azalttığını; uzun süre veya yüksek dozda uygulamanın arttırdığını göstermektedir. Ayrıca düşük dozlarda sorafenibin immün denetim noktasını hedefleyen anti-PD-1 veya anti-PD-L1 monoklonal antikorlarla birlikte kullanımının daha ileri modellerde test edilmesinin yararlı olabileceğini düşündürmektedir.tr_TR

dc.description.ozetThe Effects of Sorafenib on PDL1 Expression in Hepatocellular Carcinoma Hepatocellular carcinoma is one of the leading cause of cancer-related deaths. The most common systemic therapy for HCC is the sorafenib therapy. Although sorafenib therapy increases the median survival for a couple of months and it becomes ineffecitve after a while due to durg resistance. Elevated PD-L1 levels on HCC is associated with poor prognosis. In this study we investigated the effect of sorafenib on PD-L1 expression on HCC cell lines. In order to accomplish our purpose we studied PD-L1 levels in two different cell lines (HEPG2 ve HUH7). The cells were treated with certain doses of sorafenib (1, 5, 10 and 20 μM) for 24 hours and 72 hours. The cells were collected after the incubation and analyzed by qPCR to determine the expression of the PD-L1 gene. In additon, PD-L1 protein expression on the cell surface at 72 hours was analyzed by flow-cytometry. According to the results of qPCR, HEPG2 cells incubated by all doses for 24 hours showed less PD-L1 comparing to control, yet after 72 hours incubation there was only one statistically significant increase of PD-L1 at the dose of 20μM. Similarly HUH7 cells showed decrease of expression in cells treated with 1-10μM doses of sorafenib both after 24 and 72 hours incubation. Furthermore the results of flow-cytometry shows in HEPG2 cells, lower doses of sorafenib (1 and 5μM) induced lower PD-L1 expression compared to higher doses of sorafenib (10 and 20μM). In HUH7 cells there was no significant difference for the 10 μM dose of sorafenib, however forthe other doses there were clear increasingof the PD-L1 expressions. In conclusionour results show that short-term and small doses of sorafenib may decrease PD-L1 expression and long-term exposure to sorafenib induce the ligand expression. It could be reasonable to study sorafenib in combination with anti-PD-1 and anti-PD-L1 mAb.tr_TR
