dc.description.abstractAmaç: Bu çalışma, çocuklarda kiloluluk ve obeziteye etki eden etmenlerin belirlenmesi ve gerekli önlemlerin alınmasına katkıda bulunmak amacıyla yapılmıştır. Gereç ve Yöntem: Araştırma, Ekim 2016 – Nisan 2017 tarihleri arasında Ankara Üniversitesi Çocuk Sağlığı ve Hastalıkları Anabilim Dalı, Sosyal Pediatri Bilim Dalı polikliniğinde yürütülmüştür. Türkiye'de Okul Çağı Çocuklarında Büyümenin İzlenmesi (TOÇBİ) Araştırmasında 6-10 yaş grubunda yaklaşık %20 olarak belirtilen kiloluluk ve obezite prevalansı dikkate alınmış ve prevalansta 0.20'lik bir sapma olacağı öngörülerek %95 olasılıkla testin gücü 0.80 olduğunda, alınması gereken minimum örnek hacmi 384 olarak hesaplanmış ve çalışma grubuna 400 çocuk alınmıştır. Çalışmada 2-6 yaş (24-72 ay) çocukların kiloluluk ve obezite durumu, DSÖ tarafından DSÖ 2007 kriterlerine göre sırasıyla 2-5 yaş ve 5 yaştan büyük çocuklar için geliştirilen Anthro ve AnthroPlus paket programları ile değerlendirilmiştir. Ailelerin sosyodemografik özellikleri, annenin ve çocuğun özellikleri, çocuğun beslenme özellikleri ile ilgili bilgiler 55 sorudan oluşan veri toplama formu aracılığıyla araştırmacı tarafından annelerle yüz yüze görüşülerek toplanmıştır. Çocuk ve annenin boy ve ağırlık ölçümlerini araştırmacı yapmıştır. Verilerin analizinde, SPSS (Statistical Package for the Social Sciences) sürüm 11.5 kullanılmıştır. İstatistiklerde yüzdelik dağılım ve ortalamalar, kategorik değişkenlerin analizinde ise Ki-kare ve Fischer' in kesinlik testi kullanılmıştır. Kiloluluk ve obezite riski ile ilişkili faktörlerin değerlendirilmesinde, kilolu ve obez grupları birleştirilerek çok değişkenli lojistik regresyon analizi yapılmıştır. Bulgular: Araştırma grubunun %46.8'i kız, %53.2'si erkektir. DSÖ BKİ Z-skoruna göre %87.5'i normal; %9.3'ü kilolu ve %3.3'ü obezdir. Araştırma grubundaki çocukların anne sütü alma süresi ortalaması 18.3 ± 8.5 ay (0-48 ay) , yalnızca anne sütü alma süresi ortalaması 4.5 ± 2.6 aydır. Grubun %67.8'i ilk 6 ay yalnızca anne sütü almıştır. Çocukların kiloluluk ve obezite durumunun, annenin gebelikte ağırlık artışı durumu ile ilişkili olduğu, gebeliği boyunca 15 kilogram ve daha fazla ağırlık artışı olan annelerin çocuklarının, gebelik süresince kilo vermiş ya da 14 kilogram ve daha az ağırlık artışı olmuş annelerin çocuklarına göre daha fazla kilolu ve obez olduğu bulunmuştur. Çocukların kiloluluk ve obezite durumu, çocuğun ilk 2 yaştaki beslenme özellikleri, annenin gebelikte gestasyonel diyabet tanısı alması, annenin gebeliğinde sigara içmesi, ailede diyabet varlığı, annenin ve babanın kiloluluk durumları ile ilişkili bulunmamıştır. Çok değişkenli lojistik regresyon analizine göre çocuklarda kiloluluk ve obezite sıklığı, annenin gebeliğinde 15 kg ve daha fazla ağırlık artışı olması, çocuğun biberon kullanması, haftalık paketli gıda tüketiminin haftada birden daha sık olması ile artan yönde, çocuğun beslenme sorunu olma durumu ile azalan yönde ilişkili bulunmuştur. Sonuç: Araştırmanın sonucuna göre anne gebeliğinde fazla ağırlık artışı, çocuğun biberon kullanımı, beslenme sorunu varlığı ve haftalık paketli gıda tüketiminin, çocuklukta kiloluluk ve obezite durumunun belirleyicileri olduğu düşünülmüştür.tr_TR

dc.description.ozetObjective: This study was conducted to determine the factors affecting weight and obesity in children and to contribute to taking the necessary precautions. Material and Method: The research was conducted at the Social Pediatrics Division of Pediatrics Department of Ankara University School of Medicine between October 2016 and April 2017. In the "Monitoring Growth in School Age Children in Turkey Study", the prevalence of overweight and obesity was found approximately 20% in the 6-10 age group. Taking into consideration this finding and assuming a deviation of 0.20 from the prevalence, with the 95% probability that the test would have a power of 0.80, the minimum sample volume was calculated as 384, therefore 400 children were enrolled in the study group. Weight and obesity status of children aged 2-6 years (24-72 months) were assessed with Anthro and AnthroPlus package programs developed by World Health Organization (WHO) according to WHO 2007 criteria for children 2-5 years old and for over the age of 5 in order. The information related to sociodemographic characteristics of the families, characteristics of the mother and the child and nutritional characteristics of the child were gathered with the face-to-face interview technique by the investigator with the application of data collection form including 55 questions. For both child and mother, height and weight measurements were made by the researcher. For data analysis, Statistical Package for the Social Sciences (SPSS) version 11.5 was used. Percentage distribution and averages were used in the statistics, and Chi-square and Fischer's precision test was used in the analysis of categorical variables. For the evaluation of the factors associated with overweight and obesity risk, multivariable logistic regression analysis was performed by combining overweight and obese groups. Results: 46.8% of the children in the study group are girls and 53.2% are boys. According to the WHO BMI Z-score, 87.5% of the children were normal; 9.3% overweight and 3.3% obese. The children in the study group had an average of 18.3 ± 8.5 months (0-48 months) of breastfeeding, while the average duration of exclusive breastfeeding was 4.5 ± 2.6 months. Of the group 67.8% had exclusive breastfeeding during the first 6 months. The weight and obesity status of the children varied according to the weight gain status of the mother during pregnancy. The children of the mothers who gained weight 15 kilograms and over during pregnancy were more frequently overweight and obese when compared to the children of the mothers who lost weight or gained less than 15 kg during pregnancy. Weight and obesity status of the children were not found to be associated with the nutritional characteristics of the first 2 years of the child, gestational diabetes mellitus in the pregnancy, smoking during pregnancy, diabetes history in the family and maternal and paternal weight status. According to multivariate logistic regression analysis, weight and obesity in children were found to be directly associated with weight gain of more than 15 kg during pregnancy, bottle feeding, weekly packaged food consumption more frequently than once a week, and inversely associated with child's nutritional problems. Conclusion: According to the results of the present study, it is thought that excessive weight gain in mother's pregnancy,bottle feeding of the child, , nutrition problems and weekly packaged food consumption are determinants of childhood weight and obesity.tr_TR
