dc.description.abstractSon yıllarda klinik eczacılık hizmeti bütün dünyada hem tedavinin başarıya ulaşması hem de tedavi maliyetlerinin düşürülmesi açısından son derece önem kazanmıştır. Hipertansiyon'da tedavi; diüretikler, β-blokerler, KKB'ler, ARB'ler ve ADEİ'ler gibi farklı gruplardan ilaçlarla mono veya kombinasyon tedavisi şeklinde sürdürülmektedir. Tedavide ilk akla gelen gruplardan birisi diüretiklerdir. Diüretiklerle sürdürülen tedavilerde karşılaşılan temel sorunlardan birisi, elektrolit özellikle Na ve K düzeylerinin düşmesidir. Çok ciddi sonuçları olan bu durumun takibi ve olumsuzlukların giderilmesi açısından klinik eczacılık hizmeti son derece önem kazanmaktadır. Biz çalışmamızda, ülkemizde piyasada mevcut olan, diüretik kombinasyonlarının kullanımı ile yaklaşık bir aylık tedavi süresi sonunda, elektrolit düzeylerinde ciddi bir değişiklik meydana gelmediğini gösterdik. Hastalarda HCTZ dozunun yükselmesi ile hipokalemiye meyil olsa bile, bu istatistiksel olarak anlamlı değere ulaşmamıştır. O nedenle diüretik içeren antihipertansif kombinasyonların kısa dönemde güvenle kullanılabileceği söylenebilir. Diüretik dozlarının artması ile elektrolit bozukluklarının belirginleşebileceği yine de göz önünde bulundurulmalıdır. Klinik eczacı ve takip eden doktorun bu açıdan dikkatli olması önemlidir.tr_TR

dc.description.ozetIn recent years, clinical pharmacy services all over the world became extremely important in the terms of the success of treatment as well as the reducing the costs of therapy. In the treatment of hypertension, therapy are maintained with different group of drugs such as, Diuretics, B-Blockers, CCBs, ARBs and ACEIs in the form of mono or combination. One of the first group in thetreatment that comes to mind is diuretics. One of the main problems encountered in the continued treatment with diuretics is electrolytes especially the fall of the Na and K levels. Clinical pharmacy services becomes very important in the terms of tracking and elimination of the problemes of this issues which has very serious consequences. In our study, we showed that with the usage of the diuretic combination drugs which are available on the market in Turkey, there is no significant change in the electrolyte levels at the end of the treatment period about a month. In patients, with the rise of HCTZ dose, it did not reach statistically significant levels even if there is a hypokalemia tendency. Therefore, it can be said that antihypertensive combinations containing diuretics can be used safely, in the short terms. But it could still be considered that electrolyte disturbances can be obvious with the increase in the dose of diuretics. In this regard, clinical pharmacist and the doctors who follows the patients must be careful about the situation.tr_TR
