Deksametazon- 21 fosfat disodyumun alt gömülü yirmi yaş dişi cerrahisinde, postoperatif ödem, ağrı ve trismus üzerindeki etkilerinin araştırılması

Reserch is continues to reduce discomfort situations which may ocuur to patients after third molar surgery like pain, trismus and swelling. During my 5 years residency program in Ankara university oral and maxillofacial surgery department; we observed that edema or swelling and pain which formed after impacted 3rd molar extraction surgery bothe the patient quality of life. This study intended in reduces like these patient complaints. The physiological response of the body tissues to injury of any cause is inflammation and pain, which vary for each patient. After every surgical intervention, inflammatory mediators (prostaglandins, leukotrienes, bradykinin, platelet activating factor, etc.) are released into the tissues in response to this irritation, with a subsequent increase in vascular dilatation and permeability, causing oedema and enhancing the interstitial tissue response. Surgical removal of the mandibular third molars is the most common surgical procedure in oral and maxillofacial surgery. The aim of this study was to compare the efficacy of three different routes of administration of (DSF) dexsamethason 21 fosfat sodium (4mg/1ml) (Onadron®. İE ULAGAY İlaç sanayi Türk A,Ş) on pain, edema and trismus after impacted lower third molar surgery. İn recent studies; the application of topical corticosteroids post operative surgical operations in smaller amounts of dose seen effective and more reliable than systemic administration of corticosteroids. Double-sided symmetrical, lower impacted 3.molar indicated for extraction .(18-45 years) patients were randomly divided into 3 groups. In each group 20 patients were examined. DSF drug was administered in anatomical regions designated as one-sided in each group (submokazal, masseter muscle, masseter muscle+ submokazal). The drug has not been applied to the other side. İn order to ensure standardization the operation was made by the same surgeon using the same technique. In our study, statistically significant difference was found between test and control groups. There was no significant statistical difference between test groups. Submokazal + masseter muscle groups in compared to other test groups show significant reduction in pain, edema and trismus by mean values but statistically no signifigant differences were found. As aresult; it is observed that pain, trismus and swelling was reduced after orally local administration of DSF following 3rd molar surgery. DSF injections is effective, easy, safe, inexpensive agent and does not cause any complications or side effects.
