Rotator manşet yırtıklarında açık ve artroskopik cerrahi yöntemlerin karşılaştırılması

Arthroscopic procedures have used widely for rotator cuff surgery for the last two decades. Especially preservation of deltoid insertion and application with small incisions are the major reasons for increasing popularity of this technique. In this study the difference between open and arthroscopic rotator cuff repair were examined at middle and long term. Fifty cases who underwent open (25 cases) and arthroscopic rotator cuff repair (25 cases) in the Department of Orthopedics and Traumatology at Ankara University (İbn'i Sina Hospital) between 1998 and 2005 were evaluated. The review included demographic characteristics of the patients including age and gender, systemic diseases, previous conservative treatment, follow up periods, type of surgery, postoperative objective and subjective evaluation with Constant and UCLA shoulder evaluation scoring systems. The average Constant scores were 83.56 and 79.56 in arthroscopic and open repair group respectively. The overall average Constant score was 81.56. Statistical analysis of the results revealed no significant difference between groups according to Constant evaluation score. The patients were evaluated according to UCLA shoulder evaluation score; average values were 29.76 and 28.8 in arthroscopic and open repair group respectively. The overall average was 29.28. Statistical analysis of the results revealed no significant difference between groups according to UCLA evaluation score. In the arthroscopic repair group lesser narcotic analgesic usage, shorter hospitalization period and quicker return to normal activities were observed when compared to open repair. We have followed the superior short term results up to 21- 31 months and no significant difference was observed in pain, function, range of motion, power scores and satisfaction rates.Keywords: Rotator cuff, shoulder pathologies.
