HBsAg taşıyıcılığı diş çekimi sonrası enfeksiyon gelişmesi açısından bir risk faktörümü?

The aim of the study is, to answer the question; Do HBsAg carrier patients have a risk factor for the development of infectious complications after tooth extraction by using the acute phase reactions? active parameters, which are WBC, NC, CRP, PCT and ESR.The study was performed with the indication of tooth extraction in 40 patients,20 of them were HBsAg (+) and 20 had no systemic disease. 100 teeth were extracted; 48 of them were in HbsAg (+) patient group, 52 were in control group. Patients were classified as basic as the HBsAg and the control group, blood was drawn totally 3 times with 4 especial tubes for each patient preoperatively and postoperatively in 1-3rd days. The obtained blood were examined and statistical analysis of preoperative and postoperative laboratory test results were assessed. In addition, extracted teeth count and teeth extraction time groups were also taken for statistical evaluation.There was no significantly statistical difference in comparison of each parameters (WBC, NC, CRP, PCT, ESR) between the HbsAg(+) and control groups.In statistically comparison of measurement time above all the patients, values of the CRP and ESR in the 1st day following teeth extraction showed an increase, but in 3rd day relatively to 1st day they were decreased.In classification according to teeth extraction count, there were no statistical difference in all parameters between 1 or 2 tooth extraction and 3 or more teeth extraction groups.In the classification according to the duration of teeth extractions, in NS, CRP and ESH parameters increases were noted in the 1st day after tooth extraction relatively before tooth extraction, but in the 3rd day after tooth extraction there was decrease relatively to the 1st day following tooth extraction between the 15 and more minutes lasting teeth extractions and relatively 15 and lesser minutes lasting teeth extractions. However; increasing the tooth extraction count and extraction time increased sistemic infection temporarily.Also in clinical evaluation of each parameters among all the patients according to the laboratory test results, it is observed that in comparison of drawn blood results in the 1st and 3rd day before and after tooth extraction, values were increased, decreased or stayed the same. However, the values were decided as they were not at the pathological limits owing to the fact that there were no pathological state among all the patients.In postoperative infection follow-up period, CRP and ESR parameters were found more precise than the others (UK, NS, PCT).Although the results taken do not show very distinctive differences, it is confirmed that the results can be showed by numerical and concrete datas.Tooth extraction procedures must be done in patients with HBsAg as well as in patients without any systemic diseases, and also the specific methods of protection from hepatitis, should be applied.Key words: CRP, ESR, HBsAg, NC, PCT, Tooth extraction, Temporary Systemic Infection, WBC.
