Kardiyopulmoner bypass uygulanmaksızın direkt miyokard revaskülarizasyonu

OPCAB (?off-pump coronary artery bypass?) is a surgical approach in myocardial revascularisation without the use of cardiopulmonary bypass. İn the present study 89 patients underwent OPCAB were evaluated retrospectively. There were 9 (%10,1) women and 80 (%89,9) men with an age ranging from 42 to 87 years. 73 patients underwent a limited left anterior thoracotomy for single vessel coronary artery grafting on a beating heart. 16 patients underwent a median sternotomy, 15 of them for two vessel coronary artery grafting, one of them for single vessel coronary artery grafting on a beating heart. A total of 104 distal anastomoses were performed. There were 2 (%2,2) inhospital mortalities. There was no perioperative myocardial infarction, apoplexy. 37 (%41,5) patients with a mean of 1,4 anastomoses per patient underwent repeat coronary angiography and 100 % patency was observed. 3 patients were reexplored for hemorrhage. 5 patients required 11 blood products. 75 (%84,3) patients were discharged home 3-5 days postoperatively. Our data show that OPCAB procedures are effective and alternative therapeutic option in respect of survival, complication rates and costs in the treatment of selected patients.
