Huntington Hastalarının İzleminde Biyolojik Belirteç Olarak Nöron Kökenli Ekzozomlarda Mitokondrial DNA Düzeyi ile miR-27a ve miR-124 Ekspresyonunun Araştırılması
Özet
Huntington hastalığı istemsiz koreiform hareketler, bilişsel ve psikiyatrik
belirtilerle karakterize ilerleyici nörodejeneratif bir hastalıktır. Hastalığın
patogenezinden HTT genindeki CAG tekrar sayısının artışı sorumludur. Bu artış
nedeniyle genin kodladığı ürün olan huntingtin proteininin katlanması bozulmakta,
buna bağlı olarak nörodejenerasyonla sonuçlanan bazı süreçler başlamaktadır. Bu
süreçlerin en çok kabul görenlerinden biri de mitokondriyal disfonksiyondur.
Nörodejeneratif süreçler nedeniyle oksidatif stres artışı olduğu ve bu durumun
zamanla mitokondriyal DNA’ya hasar verdiği bildirilmiştir. Öte yandan yapılan
çalışmalarda miR-27a ve miR-124’ün düzeyindeki değişimler de mitokondriyal
disfonksiyon ile ilişkili bulunmuştur.
Bu çalışmada Huntington hastalarının serum örneklerinden elde edilen nöron
kökenli ekzozomlarda mitokondriyal hasarın göstergesi olarak mitokondriyal DNA
düzeyi incelenmiştir. Aynı zamanda miR-27a ve miR-124 düzeyleri araştırılmıştır.
Çalışmaya 35 hasta ve 20 sağlıklı birey dahil edilmiştir. Hastalığın erken ve ileri
evreleri arasında kıyaslama yapabilmek amacı ile hastalar UHDRS skorlarına göre
alt gruplara ayrılmıştır.
Hasta ve kontrol grubu arasında miitokondriyal DNA düzeyleri arasında fark
bulunamamıştır. Hasta grubu içinde yapılan kıyaslamalarda da fark
bulunamamıştır. Öte yandan, Huntington hastalığında mitokondriyal harabiyet
olduğu bilinmektedir. Bu nedenle gruplar arasında fark bulunamaması olgu
sayısının az olmasına bağlı olabilir. Daha geniş bir hasta grubunda çalışmanın
gerçekleştirilmesi halinde bu farkın gösterilebileceği düşünülmektedir.
Bildiğimiz kadarı ile literatürde nöron kökenli ekzozomlarda mitokondriyal DNA
ile ilgili yapılan bir çalışma bulunmamaktadır. Bu nedenle bu çalışmamız, nöron
kökenli ekzozomlarda mitokondriyal DNA’nın varlığını göstermekle birlikte
mitokondriyal disfonksiyon/harabiyetle seyreden çok sayıda hastalık grubunda bu
konuda çalışmaların yapılabileceğini de göstermiştir.

Summary:
Huntington disease is a progressive neurodegenerative disorder characterized by
involuntary choreic movements, cognitive decline and psychiatric disturbances.
CAG repeat expansion in HTT gene is responsable for the disorder. This repeat
expansion causes misfolding of the gene product, huntingtin protein, and triggers
some cellular processes leading to neurodegeneration. One of the most known of
these processes is mitochondrial dysfunction. Neurodegenarative processes
incerase oxidative stress and in time, this leads to mitochondrial DNA damage. On
the other hand, it has been shown that changes in miR-27a and miR-124 levels are
associated with mitochondrial dysfunction.
In this study, mitochondrial DNA level was investigated in neuron-derived

exosomes isolated from serum samples of Huntington disease patients as a marker
of mitochondrial damage. miR-27a and miR-124 levels were also investigated. 35
patients and 20 healthy individuals were included in the study. To be able to make
comparisons between patients in early and advanced stages of the disease, the
patients were divided into subgroups based on their UHDRS scores.
No statistically significant difference was found between patient and control group
for mitochondrial DNA levels. There was also no statistically significant difference
between patient subgroups, early-stage and late-stage cases. On the other hand, it is
known that there is mitochondrial degeneration in Huntington disease. Therefore,
the reason of failure in showing the difference may be due to limited number of
cases. We suggest that it is highly possible to show the difference by performing
studies in large group of patients.
To our knowledge, there is no study in the literatüre on neuron-derived exosomal
mitochondrial DNA. Therefore, this study not only show the presence of
mitochondrial DNA in neuron-derived exosomes but also shows that many studies
may be performed on this area in a vast number of diseases with mitochondrial
degeneration/dysfunction.
