Serebellum lezyonlu olgularda bilişsel işlev değişikliklerinin incelenmesi
Özet
Serebellum hacim olarak beynin %10'u kadar olmasına karşın, beyin hücrelerinin yaklaşık olarak yarısını içerir. Yalnızca motor işlevleri olduğu sanılan bu organın bilişsel işlevlere de katkıda bulunuyor olabileceği düşünülmektedir. Bu çalışma serebellumun yüksek kortikal işlevlere katkısının araştırılması amacıyla düzenlenmiştir. Bu işlevler; büyüsel düşünce eğilimi, görsel-uzaysal algı, işleyen bellek, planlama, davranış örüntüsünü değiştirebilme, uyum, sözel zeka ve performans zekasıdır. Çalışma, santral sinir sisteminin başka herhangi bir yerinde saptanabilen lezyonu olmayan, 15-55 yaş arası sağ (n= 8) ve sol (n= 10) serebellar lezyonlu toplam 18 hasta ve yaş, cins, okul yılı ve el-ayak-göz tercihleri açısından eşleştirilmiş 18 kontrol olgusu karşılaştırılarak yapılmıştır. Uzaysal algı, işleyen bellek, planlama, davranış örüntüsünü değiştirebilme ve büyüsel düşünce testleri tüm olgulara uygulanmıştır. Uzaysal algı değerlendirmesinin bir bileşeni olan iptal testi ve WAIS testleri ise 16 lezyonlu olgu ile 16 kontrol hastasına uygulanabilmiştir. Olgularda öncelikle her iki elin ince motor başarısını değerlendirmek ve motor lateralizasyonun saptanması için Parmak Vuru Testi kullanıldı. Çizgi Bölme Testi ve ptal Testi uzaysal alan ihmalinin değerlendirilmesine yöneliktir. Corsi Küp Yerleştirme Testi ise uzaysal işleyen bellek testidir. Test sonuçları serebellumun görsel-uzaysal algıya katkıda bulunduğunu göstermiştir. Corsi Küp Yerleştirme Testi bulguları işleyen bellek işlevinde belirgin bir gerilemeye işaret etmemiştir. Sağ serebellar lezyonlu olgular ile serebellar lezyonlu olguların tümünün kontrollerine göre daha yüksek büyüsel düşünce puanları aldıkları saptanmıştır. Bu bulgu sol serebellar lezyonlu olgularda yoktur. Bu sonucun bozulmuş ?kortikal-talamik-serebellar-kortikal? devreye ikincil olduğu düşünüldü. Sol hemisferin zamansal algı yeteneğinde, sağ serebellar hasara bağlı olarak ortaya çıkan gerilemenin algısal analizde ve sonuçta düşünce süreçlerinde bozulmaya yol açtığı sonucuna varılmıştır. Zeka düzeylerinin saptanmasına yönelik olarak uygulanan WAIS testi sonuçları serebellum lezyonlu olgularda özellikle sözel IQ puanlarında gerileme göstermiştir. Zeka testi puanları normaldışı inançları ölçümüne yönelik Büyüsel Düşünce puanları ile negatif korelasyon göstermektedir. Bu iki bulgu birlikte değerlendirildiğinde, serebellumun öncelikle prefrontal bölge işlevlerinin yürütülmesinde `subkortikal' devre olarak katıldığı önermesi desteklenmektedir.AbstractThe cerebellum corresponds %10 of brain?s volume though it has approximately half of the number of the brain?s cells. It was suggested to have only motor functions at first, now it is considered to participate in cognitive functions as well. This study has been organized to search for cerebellar contribution to higher cortical functions. These functions are; visual-spatial perception, working memory, planning, set- shifting, adaptation, verbal and performance IQs. The study consisted 18 patients with benign cerebellar lesions, aged between 15-55 years, who had no other central nervous system pathology. The patients were divided as right cerebellar lesion group (n= 8) and left cerebellar lesion group (n= 10). They control sample consisted of 18 healthy controls matched for age, gender, years of education, hand-foot and eye preference. The tests measuring visual-spatial perception, working memory, planning, set-shifting, and magical ideation questionnaire were done for all 18 patients and controls. Cancellation Test, which is a part of spatial perception measurement, and WAIS study were completed by 16 patients and matched controls. Finger Tapping Test was primarily used to assess fine motor performance and dominance of both hands. Line Bisection Test and Cancellation Test are used to evaluate the spatial neglect. Corsi?s Block Tapping Test is used to assess working memory. Test results confirmed the participation of the cerebellum in visual-spatial perception. However, the results of the Corsi?s Block Tapping Test did not reveal overt regression in working memory. Patients with right cerebellar lesions and cerebellar lesion group as a whole were found to have significantly higher magical ideation scores compared to the controls. This was not found in patients with left cerebellar lesions. Higher magical ideation scores were attributed to the disturbed ?cortico-thalamo-cerebello-cortical circuitry?. This was attributed to the negative effect of the right cerebellar lesion on the temporal processing ability of the left cerebral hemispere, manifesting as defective thougt processing. Wechsler Adult Intelligence Scale results showed specific decrease in verbal IQ scores in patient group. The verbal and total IQ scores were negatively correlated with the magical ideation scores in the patient group. In the control group, however, performance IQ scores were found to be negatively correlated with the magical ideation scores. When the results of the study were evaluated, the hypothesis of the cerebellum?s participation in the execution of the functions of the prefrontal cortex as a `subcortical? circuit, is being supported.
