Le Fort I Osteotomisini takiben oluşan işitsel değişikler
Özet
Le Fort I osteotomisi maksillayı içeren ortodontik veya patolojik girişimlerde sıklıkla uygulanan bir tekniktir. Maksillanın kırılması ve takiben hareketlendirilmesi şeklinde özetlenebilecek bu teknik sonrası, maksillanın anatomik komşulukları ve bu yapılara ait fonksiyonlarda değişiklikler olması muhtemeldir. Literatürde, ortognatik cerrahiyi takiben, bu anatomik bölgelerde ve bu bölgelere ait fonksiyonlardaki değişimlere ait birçok makaleye rastlanılabilir. Ancak işitsel değişikliklere yönelik halen yeterli sayıda çalışma bulunmamaktadır. Östaki tüpüne yapışan tensor veli palatini ve levator veli palatini kasları aynı zamanda maksillanın posterioruna da tutunmaktadır. Bu ilişki yüzünden Le Fort I osteotomisini takiben maksillanın hareketlendirilmesinin östaki tüpü ve dolayısıyla işitme hassasiyetini de etkileyebileceği ileri sürülebilir. Bu tez çalışması ile, bu tarz bir cerrahi işlem uygulanan hastalarda, işitme hassasiyetinde, östaki tüpünde, orta kulak basıncında ve kompliansta oluşabilecek muhtemel değişimleri araştırmak amaçlanmıştır. 12 hastada, ameliyat öncesi ve ameliyat sonrası 6'ncı haftada odyometrik ölçümler yapılarak farklılıklar araştırılmıştır. İşitme hassasiyeti için saf ses odyometre, orta kulak basıncı ve östaki tüpü değerlendirmeleri için timpanometri kullanılmıştır. Sonuçlar Le Fort I osteotomisini takiben ameliyat öncesi ve sonrası işitsel hassasiyet, orta kulak basıncı, östaki tüpü fonksiyonlarında istatistiksel olarak anlamlı bir fark sunmasa da, çalışmamızda bazı hastalarda görülen tubal fonksiyondaki iyileşmeyi daha iyi yorumlayabilmek için hasta sayısını arttırarak yeni çalışmalar yapılması gerektiğini düşünmekteyiz.AbstractLe Fort I osteotomy is a frequently used technique for orthodontic or pathologic cases which include maxilla. It is possible that some changes occur in anatomical neighborhood and the functions of these structures following the technique, which can be summarized like mobilizing the maxilla after down fracture. There are many articles about alterations on these anatomical regions and their functions in literature. But the studies on the alterations about hearing system are still inadequate. Tensor veli palatini and levator veli palatini muscles have contact with both posterior maxilla and eustachian tube. Thus it can be speculated that mobilizing the maxilla following Le Fort I osteotomy may have an affect on eustachian tube and secondarily hearing sensitivity because of this relation. The purpose of this thesis is to investigate the potential alterations of hearing sensitivity, eustachian tube, middle ear pressure, and compliance in patients undergoing this type of surgical procedure. Alterations were examined preoperatively and 6th week postoperatively on 12 patients by audiological tests. Hearing sensitivity evaluations were done by pure tone audiometry and middle ear pressure and eustachian tube functions were performed by tympanometric examinations. Although results indicated that there are no significant statistical alterations between preoperative and postoperative hearing sensitivity, middle ear pressure, and eustachian tube function following Le Fort I osteotomy, further studies have to be done with increased number of patients to explain the reason of improvement on tubal function of some patients in our study.
