Otopsi kemik dokusu örneklerinde toksik metal ve iz element düzeylerinin belirlenmesi ve metallotiyonein gen polimorfizmi ile ilişkisinin araştırılması

Çinko ve bakır gibi esansiyel metallerin yanı sıra kurşun ve kadmiyum gibi toksik metallere maruziyet endüstriyel gelişmelere paralel Çinko ve bakır gibi esansiyel metallerin yanı sıra kurşun ve kadmiyum gibi toksik metallere maruziyet endüstriyel gelişmelere paralel şekilde artmıştır. Kemik dokudaki metal birikimi de uzun süreli bu maruziyetin kanıtlarından biridir. Diğer yandan dokulardaki metal birikiminden çevresel faktörler kadar genetik yatkınlığın katkısı olabileceği düşüncesiyle yapılan bu çalışmada Ankara şehri sınırları içerisindeki genel nüfusu yansıtması adına bu bölgedeki otopsi sırasında alınan kemik örneklerinde esansiyel ve toksik metal düzeylerinin belirlenmesi ve metallotiyonein geninde polimorfik karakter ile metal düzeyleri arasında korelasyon olup olmadığının araştırılması amaçlanmıştır. Ankara Adli Tıp Kurumu'na gelen 125 ölüm vakasından 18-86 yaş aralığında olan 121 otopsi kemik dokusunda Zn, Cu, Pb and Cd düzeyleri belirlendi. Metal analizleri için örnekler göğüs kafesi -kosta kemiklerinden alındı, analiz anına kadar -20?C'de muhafaza edildi. Kuru örnekler HNO3 ile 220?C'de yakıldı ve suyla seyreltilerek analiz edildi. Zn ve Cu için Atomik absorpsiyon spektrometresi (AAS) kullanılırken Pb ve Cd tespiti için grafit fırınlı AAS sistemi kullanıldı. Aynı otopsi olgularına ait DNA izolasyonu yapılabilecek dokulardan; metallotiyonein 2A kor promotor bölgesindeki A?G tek nükleotid polimorfizmi (SNP), Restriksiyon Fragman Uzunluk Polimorfizmi (RFLP) tekniği ile belirlendi. Kemik dokuda Cu, Zn, düzeyleri sırasıyla 3,36±0,51 ppm, 27,38±1,43 ppm olarak tespit edilirken Pb ve Cd düzeyleri 437,17± 75,66 ppb ve 97,31±10,63 ppb olarak kemik dokuda tespit edildi. MT2A genindeki polimorphizm genotipleri ise homozigot tipik, heterozigot ve homozigot atipik olarak gruplandırıldı, bu genotiplerin frekansı sırasıyla %89,3, %9,9 ve %0,8 olarak hesaplandı. Cinsiyet ve kemik metal konsantrasyonları incelendiğinde, kadınlarda (711,79±243,41 ppb) erkek (320,10±54,98 ppb) bireylere oranla istatistiksel olarak anlamlı yüksek düzeyde kurşun bulundu (p<0,05). Sigara içme alışkanlıkları da kemik dokuda Cd birikmesinde önemli etken olarak anlamlılık taşıdığı; sigara kullanan bireylerin kemik kadmiyum düzeylerinin ortalaması 104,02±14,27 ppb iken içmeyenlerde bu düzeyin 64,88±8.16 ppb'ye düştüğü görüldü. Bunun yanı sıra MT2A gen polimorfizminin kemikte Cd birikimine etkidiği istatistiksel olarak belirlendi (p<0,05). Genotiplere göre homozigot tipik, heterozigot ve homozigot atipik bireylerde kemik Cd düzeyleri sırasıyla ortalama 80,11±8,19ppb, 149,86±36,00ppb ve 146,78ppb olarak bulundu.AbstractLife time exposure of toxic metals is regrettably ineluctable in today?s industrial world. Levels of metals in bone tissue are one of the evidences of this exposure. Besides the environmental factors of metal exposure and accumulations in human body, the genetic susceptibility is undeniable. The intention of this study was to determine a range of skeletal concentrations of essential (Zinc and copper) and toxic metals (Lead and Cadmium) and also to investigate polymorphism of MT2A core promoter region researching its genetic effect on metal accumulation in bone tissue for individuals? representative of the general population of Ankara district. The levels of Zn, Cu, Pb and Cd were determined in bone tissues of 121 out of 125 subjects deceased in Ankara province at the age of 18-86. Samples were collected from chess-costa bone during autopsy procedure and kept at -20?C until the analysis. Bone samples were dried for weight stability and digested with HNO3 at 220?C. The diluted samples were investigated with flame atomic absorption spectrometry for Zn and Cu. Atomic absorption spectrometer equipped with a graphite furnace and Zeeman background correction system was utilized for Pb and Cd determination. From the same autopsy cases available tisseus are collected. A ? G Single nucleotide polymorphism (SNP) was investigated in core promoter region of MT2A by PCR-RFLP technique. The levels of Cu, Zn, were 3.36±0.51 ppm, 27.38±1.43 ppm, whereas Pb and Cd levels were 404.96±69.04 ppb and 97.31±10.63 ppb in bone tissues, respectively. The genotype frequencies of the SNP were found as % 89,3 homozygote typical, % 9,9 heterozygote and % 0,80 homozygote atypical. Considerably high accumulation of lead in the bone tissues was monitored for female individuals (711.79±243.41 ppb) comparing with male (320.10±54.98 ppb) (p<0.05). Significant positive correlation was shown in between age and zinc levels. Statistically significant positive correlation was observed between the A? G SNP in the MT2A and the cadmium levels of the individuals (p<0.05). It is also found that smoking habit is one of the most effective parameter for Cd accumulation in bones; significantly high Cd and Zn levels were measured for the smokers? bone tissues. Cd average values were found as 104,02±14,27 ppb for smokers where as 64,88±8.16 ppb for non-smokers (p<0.05).
