Adölesan gebelerde beslenme durumu ile yenidoğan ağırlığının değerlendirilmesi

Gözlemsel ve kesitsel nitelikte bir izlem çalışması olan bu araştırma, adölesan gebelerin beslenme durumları, beslenme alışkanlıkları, ağırlık kazanımları, hematolojik ve biyokimyasal düzeyleri ile yeni doğan ağırlığının araştırılması amacıyla, Konya’da, Necmettin Erbakan Üniversitesi Meram Tıp Fakültesi Hastanesi Kadın Doğum Polikliniği’ne başvuran, 16–19 yaşlarında, çalışma şartlarına uygun 32 adölesan gebe (ortalama yaş: 18.31±0.8yıl) ve yenidoğan üzerinde yürütülmüştür. Araştırmanın verileri anket tekniği ile toplanmıştır. Gebelerin 3 günlük besin tüketimleri, tüketim sıklıkları, beslenme alışkanlıkları, bazı antropometrik ölçümleri ile hematolojik ve biyokimyasal düzeyleri, üç trimester süresince kaydedilmiştir. Ayrıca, yeni doğanın genel bilgileri, sağlık durumu, beslenme şekli doğum boyu, vücut ağırlığı ve baş çevresi ölçümleri kaydedilmiştir. Yenidoğanların ağırlık kazanımlarını belirlemek amacıyla, dört hafta süreyle vücut ağırlığı ölçümleri tekrarlanmıştır. Araştırmanın sonucunda, gebelerin gebelik süresince toplam ağırlık kazanımlarının ortalama 12.0±5.4 kg olduğu; I.trimesterde % 50.0’sinin, II.trimesterde % 71.9’unun, III.trimesterde % 43.8’inin hemoglobin düzeylerinin düşük olduğu saptanmıştır. Gebelerin çoğunluğunun enerji alımlarının, üç trimesterde, yeterli olduğu (sırasıyla; % 87.5,% 90.6,% 93.8); demir, kalsiyum, niasin, folik asit, fosfor ve B12 alımlarının yetersiz olduğu saptanmıştır. Gebelerin I.trimesterde % 53.1’inin; II. ve III.trimesterlerde % 78.1’inin günde 3 ana öğün tükettikleri belirlenmiştir. Trimesterlere göre, gebelerin süt ve ürünleri (p:0.039), et, yumurta (p:0.024), kuru baklagil, yağlı tohumlar, tahıllar, yeşil yapraklı sebzeler (p:0.016), turunçgiller, diğer meyveler, margarin, şeker ve şekerli besin tüketimlerinin arttığı belirlenmiştir. Çalışmada, 1 bebeğin 2500 g’ın altında doğduğu, yenidoğanların doğum ağırlığının ortalama 3172±361.4 g, boy uzunluğunun 50.1±1.1cm, baş çevresinin 34.6±0.5 cm olduğu, 1.ayın sonunda, toplam ağırlık kazanımlarının ortalama 767.5±126.5g olduğu saptanmıştır. Gebelerin I., II. ve III.trimester vücut ağırlıkları (sırasıyla; rs= -0.36,-0.43,0.43, p<0.05) ve BKI’leri (sırasıyla; rs= 0.36,0.12,0.39, p<0.05) ile yeni doğanların doğum ağırlığı arasında pozitif yönde anlamlı ilişki bulunmuştur. Ayrıca, yenidoğanların doğum ağırlığı ile gebelerin III.trimester hemoglobin ve hematokrit düzeyleri arasında negatif yönde anlamlı ilişki bulunmuştur (sırasıyla; rs= -0.43,-0.35, p<0.05).AbstractThis follow-up study, designed as a observational and cross-sectional research, was conducted in order to determine the nutritional status, nutritional habits, weight gains, hematological and biochemical levels of adolescent pregnants and weights of newborns on 32 adolescent pregnants, (mean age: 18.31±0.8 years) who applied to Konya Necmettin Erbakan University, Meram Medical Faculty Hospital, Maternity Clinic, at the age of 16-19 suitable for the study, and their newborns. Data were collected through questionnaire technique. Three-day food consumption records, food consumption frequencies, nutritional habits, certain anthropometric measurements, hematological and biochemical levels of pregnant women were recorded during each trimester. Additionally, general information, health status, diet, birth, height, weight and head circumference measurements of newborns were recorded. In order to determine weight gains of the newborns, their body weight measurements were repeated for four weeks.At the end of the study, it was determined that the average total weight gains of pregnant women during pregnancy was 12.0±5.4 kg and hemoglobin levels of 50% in I. trimester, 71.9 % in II. trimester, 43.8 % in III. trimester of the pregnants were found to be under the references. It was determined that the majority of pregnants consumed energy at adequate levels during each trimesters (87.5 %, 90.6 %, 93.8 % respectively); their iron, calcium, niacin, folic acid, phosphorus, vitamin B12 intakes were insufficiently. It was determined that 53.1% of pregnant women in I. trimester and 78.1 % of them in II. and III. trimesters had 3 main meals a day. According to trimesters, it was determined that milk and milk products (p:0.039), meat, eggs (p:0.024), legumes, oil seeds, grains, leafy vegetables (p:0.016), citrus fruits, other fruits, margarine, sugar and sugary consumptions of the pregnant women increased. In the study, it was determined that one of the newborns was born under 2500 g. The average birth weight of newborns was 3172±361.4 g, average length was 50.1±1.1cm and head circumference was 34.6±0.5 cm and the average total weight gain was 767.5±126.5 g at the end of the first month. There was a significant and positive correlation among I, II. and III. Trimester weights of the pregnants (respectively; rs= -0.36,-0.43,0.43, p<0.05) and BMI measurements (respectively; rs=0.36,0.12,0.39, p<0.05) with birth weights of the newborns. Also, there was a significant and negative correlation between hemoglobin and hematocrit levels of the pregnant and birth weights of the newborns (respectively; rs=-0.43,-0.35,p<0.05).
