Bazı dezenfektanların nozokomiyal infeksiyon etkeni olan bazı bakteriler üzerine etkilerinin karşılaştırılması

Nozokomiyal infeksiyonlar çağımızın en önemli sorunlarından biridir. Bunlar, hastaneye yatış sırasında inkübasyon döneminde olmayan, yatıştan 48?72 saat sonra hastanede ya da taburcu olduktan sonra 10 gün içinde gelişen infeksiyonlardır. Bu infeksiyonlar; klasik infeksiyon hastalıklarına oranla daha ağır seyreder ve tedavisi daha güç infeksiyonlardır. Nozokomiyal infeksiyonlara neden olan bakterilerin çoğunun dezenfektan maddelere maruz kalmalarına rağmen canlılıklarını koruyabilen bakteriler olması, her hastanede kullanılan dezenfektanların etki spektrumlarının değerlendirilmesinin önemini göstermektedir. Bu çalışmanın amacı; hastanelerde sık kullanılan bazı dezenfektanlardan %2 gluteraldehit, %4 klorheksidin glukonat, %7,5 polyvinylprolidon, %10 polyvinylprolidon, %70'lik 2-propanol, %3 hidrojen peroksit içeren solüsyonların, Ankara Üniversitesi, Tıp Fakültesi, İbn-i Sina Hastanesi, Merkez Laboratuvarı'nca nozokomiyal infeksiyon etkeni olarak soyutlanan E.coli (26), K.pneumoniae(29), P.aeruginosa (30), S.aureus (30) ve Enterococcus sp.(21) suşları üzerindeki etkilerinin kantitatif süspansiyon testi kullanılarak, 3?5 ve 10'ar dakikalık temas sürelerinde araştırılmasıdır. Çalışmamızda kullandığımız nozokomiyal infeksiyon izolatlarının tamamı %2 gluteraldehit, %4 klorheksidin glukonat, %7,5 polyvinylprolidon, %10 polyvinylprolidon ve %70'lik 2-propanol'e karşı bütün temas sürelerinde duyarlı bulunmuştur. Ancak %3 hidrojen peroksit solüsyonu, 3?5?10 dakikalık temas sürelerinde dört E.coli, on dokuz K.pneumoniae, on iki S.aureus, üç Enterococcus sp. suşuna karşı bakterisidal etki gösterirken, yedi E.coli, on K.pneumoniae, on bir S.aureus, yedi Enterococcus sp. suşuna karşı 3 dakikada etkili olmamış, 5 ve 10 dakikalık temas süreleri sonunda etkili olduğu gözlenmiş, onbir E.coli, altı S.aureus, üç Enterococcus sp. suşuna karşı 3 ve 5 dakikalık temas sürelerinde etki göstermezken, 10 dakikalık temas süresi sonunda etkili olmuştur. %3 hidrojen peroksit solüsyonu dört E.coli, bir S.aureus, sekiz Enterococcus sp. suşuna karşı 3?5?10 dakikalık temas sürelerinin hiçbirinde etki göstermemiştir ve bu suşlar %3 hidrojen peroksite karşı dirençli bulunmuştur. Değişik hastane ortamlarından izole edilen bakterilerin duyarlılıkları farklı olabileceğinden ve sık kullanılan dezenfektanlara karşı direnç gelişebileceğinden, nozokomiyal infeksiyonların kontrolünde her hastanenin kendi mevcut verilerine uygun dezenfektanları saptayarak kullanmasının faydalı olacağı sonucuna varılmıştır. AbstractNosocomial infections are one of the most important problem of our age. These infections are not present or incubating at the time of hospital admission, they occur more than 48-72 hrs after admission or within 10 days after discharge from the hospital. These are more severe than the classical infectious diseases and more difficult to treat. Most bacteria that cause nosocomial infections can retain their viability even after exposure to disinfectants. Therefore, evaluation of the efficacy of disinfectants at each hospital is important. The purpose of this study is, to investigate the effects of widely used disinfectants (chlorhexidine gluconate (4% wt/vol), glutaraldehyde (2% wt/vol), 2-propanol (70% vol/vol), polyvinylpyrrolidone (7,5% wt/vol), polyvinylpyrrolidone (10% wt/vol), hydrogen peroxide (3% wt/vol)) on nosocomial isolates of E.coli (26), K.pneumoniae (29), P.aeruginosa (30), S.aureus (30) and Enterococcus sp. (21). Nosocomial isolates were obtained from Ankara University, School of Medicine, Ibn-i Sina Hospital, Central Microbiology Laboratory. Study was performed with quantitative suspension test, at contact times of 3-5-10 minutes.. According to the results, all the isolates were susceptible to chlorhexidine gluconate (4%), glutaraldehyde (2%), 2-propanol (70%), polyvinylpyrrolidone (7,5%), polyvinylpyrrolidone (10%) at contact times of 3?5?10 minutes. However, for E.coli only 4 isolates were found susceptible to hydrogen peroxide (3%) at contact times of 3-5-10 minutes, 7 isolates were found susceptible at contact times of 5-10 minutes, 11 isolates were found susceptible at contact time of 10 minutes and 4 isolates were resistant to hydrogen peroxide (3%). For K.pneumoniae 19 isolates were found susceptible to hydrogen peroxide (3%) at contact times of 3-5-10 minutes, 10 isolates were found susceptible at contact times of 5-10 minutes. For S.aureus, 12 isolates were found susceptible to hydrogen peroxide (3%) at contact times of 3-5-10 minutes, 11 isolates were found susceptible at contact times of 5-10 minutes, 6 isolates were found susceptible at contact time of 10 minutes and only 1 isolate was resistant to hydrogen peroxide (3%). For Enterococcus sp., 3 isolates were found susceptible to hydrogen peroxide (3%) at contact times of 3-5-10 minutes, 7 isolates were found susceptible at contact times of 5-10 minutes, 3 isolates were found susceptible at contact time of 10 minutes and 8 isolates were resistant to hydrogen peroxide (3%). Due to differences between hospital environments and susceptibility of the isolated bacteria, resistance to commonly used disinfectants may develop, each hospital should define its own disinfectant usage policy.
