Aşırı atrofik mandibulada farklı implant tasarımlarının 3 boyutlu sonlu elemanlar analizi ile değerlendirilmesi

İleri derecede atrofik alveolar kretlerde protetik rekonstrüksiyon problemini çözebilmek için farklı tedavi yaklaşımları bulunmaktadır. İleri cerrahi uygulamalarını takiben kemik içi implant yerleştirme protokolü daha travmatik bir yöntem olmakla birlikte alternatif bir tedavi şekli olarak rutin bir şekilde uygulanmaktadır. Diğer yöntemler ise son yıllarda yaygınlaşan kısa implantlar ve günümüzde kemik içi implantların gerisinde kalmış subperiostal implantlardır. İmplant üstü overdenture uygulamalar protezin retansiyonunu ve stabilizasyonunu düzelterek hasta konforunu arttırmaktadır. İmplant üstü overdenture protezlerde en çok kullanılan tutucu sistemleri bar, ball ve locator tutucu sistemleridir. Doğru tutucu tipinin seçimi, kemik ve diğer destek dokuların korunması, protez retansiyonunu arttırmak ve protezin uzun dönem başarısını sağlamak için önemlidir. Çalışmamızın amacı mandibular kretin aşırı derecede rezorpsiyona uğradığı durumlarda alternatif tedavi seçeneklerini (ogmentasyon prosedürleri, kısa implantlar ve subperiostal implantlar), farklı tasarıma sahip implantlar ile farklı tip tutucuların overdenture protez desteği olarak kullanıldığı durumlarda, kuvvet karşısında kemik ve implantlar üzerindeki stres etkilerini 3 boyutlu sonlu elemanlar analizi ile karşılaştırmalı olarak değerlendirmektir. Çalışmamızda ileri derecede atrofiye olmuş tam dişsiz alt çene kemiği ve atrofiye uğramış mandibulanın greft kemiği ile ogmentasyonunu temsil eden alt çene kemiği olarak 2 farklı kemik modellenmiştir. Bu modellere kanin ve birinci premolar bölgelerine olmak üzere farklı boyut ve tasarımlarda implantlar yerleştirilerek bar, ball ve locator tutuculu overdenture'lar hazırlanmıştır. Subperiostal implant ball tutuculu overdenture olarak düzenlenmiştir. Her modele 100 N vertikal ve 100 N açılı kuvvet uygulanmış, oluşan stresler Algor Fempro programı ile incelenmiştir. Çalışmamızın sonuçlarına göre, en yüksek stresler subperiostal implanttaki kortikal kemikte görülmüştür. Kısa implantlar, uzun implantlarla yaklaşık olarak benzer değerde stresler oluşturmuşlardır. Tasarım farklılığına bakıldığında belirgin bir fark olmamakla birlikte Bicon implant tasarımı daha olumlu sonuçlar vermiştir. Subperiostal implantın kortikal kemik üzerinde yarattığı stresler kemik içi implantlardan yüksektir. Kortikal kemikteki en düşük stresleri bar tutucular oluşturmuştur, bunu sırasıyla locator ve ball tutucular izlemiştir. İmplantlar üzerindeki stres değerlerine bakıldığında ball tutucunun daha fazla stres oluşturduğu ve bar tutucuların implantlar arası stres dağılımının daha uygun olduğu gözlenmiştir. Oblik kuvvetler, vertikal kuvvetlerden daha fazla stres yaratmaktadır.AbstractThere are number of different treatment plans for the prosthetic rehabilitation of severely atrophic alveolar ridges. Although the placement of endosseous implants is aggresive way of treatment, it has been used rutinely. The other options include, using short implants, which has become popular in recent years, or not so popular subperiosteal implants. Implant overdenture prosthesis enhances retantion and stabilization, therefore increasing the patient comfort. Ball, bar or locator attachment systems are the popular ones which are used for the implant overdenture prosthesis. Choosing the correct attachment is important for the preservation of the bone and surrounding tissues, as well as maintaining the long term success of the prosthesis. The aim of this study is to analyze and compare the stress forces on the implants and the bone at severely atrophic mandible treated with alternative treatment protocols (augmentation procedures, short implants and subperiosteal implants) using different implant designs and attachment systems in overdenture prosthesis. In this study, 2 different bone models were generated to represent severely atrophic edentulous mandible with and without bone augmentation procedures. Implants with different designs and sizes were virtually placed to the canine and 1st premolar region and overdentures with bar, ball or locator attachment systems were created. Subperiosteal implant was used with ball attachments. 100 N vertical and 100 N oblique forces were applied and the stresses created were analyzed with Algor Fempro programme. According to the results of the study, the highest stress values were recorded in the subperiosteal implant's cortical bone. Short implants had similar results with long implants. About the design consideration, there were no significant difference, however Bicon implants showed slightly higher performance. Stresses created with subperiosteal implant was higher than endosseous implants. Lower stress at the cortical bone were created by bar attachment and this is followed by locator and bar attachments. Considering the stress amount on the implants, ball attachments seem to create more and bar attachment's distribution of stress was favorable. Oblique forces created more stress values than vertical forces.
