Sabit tedavi ile meydana gelen keser konum değişikliklerinin dil konumuna etkisi

Dt. Aylin Gümüş Sabit Tedavi ile Meydana Gelen Keser Konum Değişikliklerinin Dil Konumuna Etkisi Çalışmanın amacı, sabit ortodontik tedavi ile dil konumunda meydana gelen değişimleri, değiştirilen alt kesici diş konumu ile dil konumu arasında bir ilişki olup olmadığını ve bu ilişkinin niteliğini araştırmaktır. Edgewise sabit tedavi tekniği ile tedavi edilmiş, konjenital ve/veya akkiz diş eksikliği ve ön ve/veya arka çapraz kapanışı olmayan, geç dönem karma ve/veya daimi dentisyondaki 34 bireyin tedavi başı ve sonunda kaydedilmiş lateral sefalometrik filmleri ve modelleri değerlendirilmiştir. Sefalometrik filmlerden total grubun (n=34), tedavi başı ve sonu 1-NB değerlerin farkları alınmış, retrüzyon gösteren bireylerden; retrüzyon grubu (n=10) ve protrüzyon gösteren bireylerden ise; protrüzyon grubu (n=10) olmak üzere alt gruplar oluşturulmuştur. Dil görüntüleri AccuTab GTCO sayısallaştırma masasında sayısallaştırılmış ve Microstation 95 programı ile dil alanı ölçülmüştür. Dilde sefalometrik olarak meydana gelen konumsal ve boyutsal değişimler Rakosi dil analizi kullanılarak değerlendirilmiştir. Tedavi başı sefalometrik ölçümler okluzal düzleme S noktasından dik ve paralel olarak çizilen horizontal ve vertikal referans düzlemlerine yapılan direkt ölçümlerle, tedavi sonu ölçümler referans düzlemlerinin yapısal total çakıştırma yöntemi ile taşınması ile yapılmıştır. Mandibuler ve dile ait yapılar tedavi sonu filmlere mandibuler lokal yapısal çakıştırma yöntemi ile aktarılarak değerlendirilmiştir. Dental model ölçümleri dijital pergel ve Korkhaus pergeli ile yapılmıştır. Tanımlayıcı istatistikler, student t-testi, eş yapma t-testi ve Pearson korelasyon analizi uygulanmıştır. Bulgularımız; dil ucunda alt keserde sagital yönde yapılan harekete benzer yönde ve şekilde hareket olduğu, retrüzyon grubunda dil dorsumunun yükseldiği, dil tabanının alçaldığı, protrüzyon grubunda dil dorsumunun belirgin olarak yükselmediği ve dil tabanının belirgin düzeyde alçalmadığı, hyoid kemiğinin retrüzyon grubunda belirgin derecede aşağıda, protrüzyon grubunda belirgin derecede önde konumlandığı şeklindedir.AbstractThe Influences of Incisor Positional Changes Due to Fixed Appliance Therapy on Tongue Position Aim of this study is; to examine the positional changes of the tongue due to fixed appliance therapy, the relationship between the positional changes of lower incisor and the tongue and the attribution of this relationship. Cephalometric radiographs and dental casts of 34 individuals? pretreatment and treatment records were evaluated. Pretreatment inclusion criteria were; only edgewise fixed appliance therapy, absence of both anterior and/or posterior crossbite and cases at late mixed dentition and/or permanent dentition stages. Lateral cephalometric radiographs of total group (n=34) were evaluated and the differences of 1-NB parameters were recorded. In line with the results, the subgroups namely retrusion group (n=10) and protrusion group (n=10) were formed. Tongue images were digitized on an AccuTab GTCO table and the tongue area were calculated with the Microstation 95 program. The positional and dimensional changes of the tongue due to therapy were evaluated using Rakosi tongue analysis. Pretreatment cephalometric measurements were performed from horizontal and vertical reference planes constructed from the horizontal and vertical lines passing from point S to occlusal plane. Posttreatment cephalometric measurements were evaluated with the transportation of reference lines using the total structural superimpositional method. Cast measurements were done with electronic digital compass and Korkhaus bow divider. Descriptive statistics, student t-test, paired t-test and Pearson correlation analysis was performed. The results of the study were; positional changes of the tip of the tongue were concordant with the positional changes of the lower incisor, elevation of the dorsum of the tongue and stoopness of the base of the tongue occurred in retrusion group. No statistically significant elevation of the dorsum of the tongue and stoopness of the base of the tongue was found in protrusion group. Hyoid bone was found to be lowered in retrusion group, whereas it was found to have moved forward in protrusion group.
