Farklı rotasyon modellerine sahip sınıf II,1 malokluzyonlu bireyler ile sınıf ı bireyler arasındaki dentoalveolar ve kraniyofasiyal yapı farklılıklarının değerlendirilmesi
Özet
Bu çalı şmanın amacı Türk toplumunda farklı mandibular rotasyon modellerine sahip Sınıf II,1 malokluzyonlu bireylerin, hangi morfolojik farklılıklarla birbirinden ve normal bireylerden ayrıldı ğını tespit etmektir. Bu amaçla çalı şmaya, sagittal yönde, iskeletsel olarak Sınıf II, dental olarak artmı ş overjetli, vertikal yönde ise üç ayrı mandibular rotasyon modeli gösteren bireyler dahil edilmi şlerdir. Ara ştırma 4 grup üzerinde yürütülmü ştür. İlk 3 grup Sınıf II,1 bireylerden olu şan çalı şma grupları, IV. grup ise normal okluzyonlu bireylerden olu şan kontrol grubudur. Kontrol grubu bireylerin ANB açıları 0–4° arasında ve overjet, overbite ili şkisi normal sınırlar içerisinde olup, Sınıf I dental okluzyona ve kabul edilebilir çene yüz esteti ğine sahiptirler. Çalı şma grubunda yer alan toplam 132 bireyin, 99’u Sınıf II,1 malokluzyonlu olup, ANB açıları 5° veya daha büyük, overjet miktarları, 5 mm veya daha yukarısındadır. Ayrıca çalı şma grubundaki Sınıf II,1 malokluzyonlu bireyler vertikal yönde de SN/GoGn açılarına göre Hipodiverjan, Normodiverjan, Hiperdiverjan olarak 33’er ki şilik 3 gruba ayrılmı ştır. Bireylerin gruplar içerisindeki iskeletsel geli şim safhaları, el-bilek kriterleri esas alınarak denkle ştirilmi ştir. Ara ştırmada, gruplar arasında iskelet ve dentoalveolar yapılara ait ölçümlerin farklı olup olmadıklarını test etmek amacıyla Varyans analizi kullanılmı ş, gruplar arasında farklılık bulundu ğunda, farklılı ğın kayna ğını tespit etmek amacıyla Duncan testi uygulanmı ştır. Sınıf II,1 malokluzyonlu bireyler, vertikal yönde farklı rotasyon modellerine sahip olmalarına ra ğmen, üst yüz yüksekli ği, total mandibular boyut, mandibular korpus boyutu ve alt posterior alveolar yükseklikler bakımından benzerlik göstermi ştir. Sınıf II,1 malokluzyona sahip Hiperdiverjan grupta, aslında ramus boyutu kısa olmasına ra ğmen, total mandibular boyut alt gonial açıdaki artı ş sebebiyle di ğer Sınıf II gruplarla benzer bulunmu ştur. Ancak bu boyutların tamamının Kontrol grubuna göre daha dü şük oldu ğu tespit edilmi ştir. Ayrıca, Sınıf II bireylerin simfiz morfolojileri, hem kendi aralarında hem de Kontrol grubuna göre farklılık göstermi ş, bu farklılıkların ise, özellikle simfizin üst kısmında belirgin oldu ğu saptanmı ştır. Ortaya çıkan bir di ğer bulgu ise, Sınıf II malokluzyonlarda, mandibular rotasyon modellerine göre, dentoalveolar kompenzasyonların görülmesidir. Mandibulanın anterior rotasyon modeli gösterdi ği Sınıf II bireylerde maksillar posterior ve anterior alveolar boyutların ve keser protrüzyonlarının azaldı ğı, posterior rotasyon modeli gösteren bireyler de ise maksillar posterior ve anterior alveolar boyutların ve keser protrüzyonlarının arttı ğı, overbite miktarının azaldı ğı tespit edilmi ştir. Ayrıca bu dentoalveolar de ğişikliklerin alt anterior yüz yüksekliğindeki artı ş ve azalmalarla paralellik gösterdiği, alt posterior yüz yüksekliğindeki artı ş ya da azalmalarla ise ters orantılı oldu ğu saptanmı ştır. Bununla beraber gruplar arasındaki iskeletsel farklılıklar Sınıf II,1 malokuzyonlarda fasiyal esteti ği etkileyen en önemli sebep olarak kar şımıza çıkmaktadır. Sınıf II,1 malokluzyona sahip bireylerin mandibular rotasyon modelleri dikkate alındı ğında, vertikal yön bozukluklarının sagittal maksillomandibular ili şkiler kadar önemli oldu ğu ve tedavi planlamalarında bunun dikkate alınması gerekti ği söylenebilir. AbstractThe aim of this study is to determine morphological differences among Class II,1 subjects with different rotation models and normal subjects in Turkish population. For this purpose, subjects having sagittally skeletal Class II relationship with increased overjet, presenting three different mandibular rotation models in vertical direction were included in this study. The study was composed of four groups. The first three groups were study groups that contained Class ll, 1 subjects while the fourth group was the control group consisting subjects with normal oclusion. The subjects in the control group had acceptable facial esthetics, exhibiting Class l dental occlusion, ANB angles between 0–4°, ideal overjet and overbite relationships. The study consisted of 132 subjects. Ninety-nine of these subjects, had Class ll,1 malocclusion, ANB angle equal or greater than 5° and overjet equal or greater than 5 mm. The rest made up the control group. Subjects in the study group, based on their SN/GoGn angle, were divided into three groups, as Hypodivergent, Normodivergent and Hyperdivergent, with 33 subjects in each group. Subjects in the groups were balanced according to skeletal development stages utilizing hand-wrist radiographic criteria. Variance analysis was used to assess the presence of differences in the measurements of skeletal and dental structures among the groups, and wherever a difference was present, Duncan test was utilized to determine its source. Subjects with Class II, 1 malocclusion, despite the presence of different rotation models vertically, have depicted similarities with regard to upper face height, total mandibular size, mandibular corpus size, and posterior alveolar height. Although ramus size was shorter in Class ll, 1 Hyperdivergent group, total mandibular size was found similar to other Class ll groups due to increased gonial angle in Hyperdivergent group. However, all these measurements were found smaller relative to the control group. In addition, symphysis morphology of Class ll subjects has shown differences within the study groups and from the control group. These differences were particularly found in the upper segment of symphysis. Another finding of the study was the presence of dentoalveolar compensatory mechanism depending on mandibular rotation models in Class ll malocclusion subjects. While decreased maxillary anterior and posterior alveolar dimensions and incisor protrusion were found in Class ll subjects exhibiting anterior mandibular rotation model, increased maxillary anterior and posterior alveolar dimensions, incisor protrusion and decreased overbite were found in Class ll subjects exhibiting posterior mandibular rotation model. Also, these dentoalveolar changes were in parallel with increase or decrease in lower anterior face height and were inversely related with changes in lower posterior face height. Furthermore, skeletal differences between the groups appeared to be the most important factor affecting facial esthetics in Class ll, 1 malocclusions. Regarding mandibular rotation models in Class ll, 1 subjects, anomalies in vertical direction should be regarded as important as sagittal maxillo-mandibular relations and this fact should be considered during treatment plannings.
