Talasemi hastalarında DMT1 polimorfizminin kan kurşun ve demir düzeyleri ile ilişkisinin araştırılması

Çalışmamızın amacı Talasemili hastalarda DMT1 IVS4+44 C/A Tek Nükleotid Polimorfizmini Belirlemek ve bu polimorfizmin Fe ve Pb düzeyleri ile ilişkinin olup olmadığını tespit etmektir. Talesemi kalıtsal (genlerin ebeveynlerden çocuklara aktarılması) kan hastalıkları içerisinde yer alan, hemoglobin molekülünün üretimi süresince, globin zincir sentezlerindeki düzenlenme dengesizliklerine dayalı, globin zincir sentez bozukluğudur. Dengesiz, globin zincir sentezi düşük hemoglobin üretiminin başlıca nedenidir ve anemiye neden olur. Talesemi (özellikle beta-talasemi), en önemli küresel sağlık sorunlarından biridir ve Akdeniz, Afrika. Hindistan ve Güney Asya'da yaygın olarak bulunmaktadır. Beta talaseminin ana nedeni ß-globin sentezinin yetersiz olması dengesiz globin zincir üretimine ve ?-zincirlerinin aşırı birikimine neden olur. Çalışmanın ilk bölümünde, 50 talasemili bireyde DMT1 C?A tek nükleotid polimorfizmi (SNP) araştırıldı. İzole edilen DNA'ların DMT1 bölgesi, PCR tekniği ile çoğaltılarak, amplifiye edildi. PCR ürünü amplifiye edilen oligonükleotid'ler RFLP tekniği ile kesildi. Talasemili hastalarda DMT1 tek nükleotid polimorfizmin genotip frekansları; % 30.0 homozigot tipik, %.46,0, heterozigot % 46.0 homozigot atipik genotip % 24.0 olarak belirlendi. Alel frekansı C aleline sahip bireylerde A(+) %53, C aleline sahip olmayan bireylerde A¬(-) %47 olarak tespit edildi. İkinci bölümde ise; DMT1 C?A tek nükleotid polimorfizmi belirlenmiş olan 50 kan örneğinin Fe, Pb konsantrasyonları ölçüldü ve polimorfik özellikle metal düzeyi arasında ilişki olup olmadığı araştırıldı. Fe için Atomik absorpsiyon spektrometresi (AAS) kullanılırken Pb tespiti için grafit fırınlı AAS sistemi kullanıldı. Çalışma sonucunda, demir düzeyi homozigot genotiplerde (1021.68±113.90 ppm) heterezigot (884.32±208.07 ppm) ve homozigot atipik genotiplere (784.55±181.47 ppm). göre daha yüksek olarak tespit edildi (p=0,002). Ancak, Kurşun düzeyi homozigot genotiplerde (31.26±8.01 ppb) heterezigot (33.23±6.11 ppb) ve homozigot atipik genotiplere (53,84±28.82 ppb). göre daha düşük tespit edildi (p=0,013). C aleline sahip bireylerde A(+) demir düzeyi (850.11±202.43 ppm), C aleline sahip olmayan bireylere A¬(-) göre düşük olduğu tespit edildi (1021.68±113.90 ppm) (p=0,001). Kurşun düzeyi C aleline sahip olmayan bireylerde A(-) (31.26±8.01 ppb) , C aleline sahip bireylere (40,29±19,78 ppb) (p=0,048) göre daha düşük çıkmıştır. Çalışmamızda demir ve kurşun arasında negatif korelasyon gözlemlenmiştir ancak bu sonuç istatistiksel olarak anlamlı değildir.AbstractDetermination of iron and lead levels in thalassemic patients blood samples and investigation of its relation between DMT1 gene polymorphisms. The aim of this study is to determine the polymorphism of DMT1 in thalessemic patients and investigate whether this polymorphism has an effect on Fe element levels and Pb metal levels in the blood samples. Thalassemias are hereditary blood diseases (that is, they're passed from parents to children through the genes) due to unbalanced globin chain synthesis. Unbalanced globin chain synthesis is the major cause of low level hemoglobin production leading to anemia. The Thalassaemias (especially beta-thalassemia) pose by far the most important global public health problem which are widespread throughout the Mediterranean region Africa, the Middle East, the Indian subcontinent, and Southeast Asia. Main cause of the beta thalassemias is defective ß-globin synthesis, which leads to imbalanced globin chain production and an excess of ?-chains. For the first part of the this study, determine genetic polymorphism of the DMT1 were analyzed in 50 blood samples. C?A single nucleotide polymorphism (SNP) was investigated in DMT1 IVS4+44 region C/A extraction DNAs, were amplified with the PCR technique. Then, the amplified oligonucleotides were cut with the RFLP technique. The genotype frequencies of the SNP in the patients DMT1 IVS4+44 region C/A were determined as follows; 30.0% homozygote typical (CC), 46.0% heterozygote (CA) and 24.0% homozygote atypical (AA) genotype. Additionally the frequency of C allele A(-) was found as 53.0% and of A allele A(+) as 47.0%. For the second part of this study, determination of C?A SNP in the DMT1, the Fe and Pb, levels of the 50 blood samples were measured and the correlation between the polymorphic characteristics and metal level were investigated. Lead levels were measured by Varian AA240Z Zeeman Graphite Furnace Atomic Spectrometer, Iron levels were measured by Varian AA240FS Fast Sequantial Flame Atomic Absorbtion Spectrometer. Iron levels are higher in homozygote genotype (1021.68±113.90 ppm) than heterozygote genotype (884.32±208.07 ppm) and homozygote atypical genotype (784.55±181.47 ppm). This is statistically significant (p=0,002). On the other hand Leads levels in homozygote genotype (31.26±8.01 ppb) are lower than heterozygote genotype (33.23±6.11 ppb) and homozygote atypical genotype (53,84±28.82 ppb). This finding is also statistically significant (p=0,013). Iron levels are significantly lower in A(+) allele (850.11±202.43 ppm) than in A(-) allele (1021.68±113.90 ppm) (p=0,001). Furthermore, lead levels are statistically significant lower in A(-) allele (31.26±8.01 ppb) than in A(+) allele (40,29±19,78 ppb) (p=0,048). In our study data show that negative correlation between iron and lead. However, this result is not statistically significant.
