Park Sağlık Ocağı bölgesinde 15-49 yaş evli kadınlarda gebeliği önleyici yöntem kullanımı ve ilgili etmenler
Özet
Bu araştırma, Park Sağlık Ocağı Bölgesi'nde 15-49 yaş grubu evli kadınlarda, gebeliği önleyici yöntem kullanımı ve ilgili etmenlerin ilişkisini belirlemek amacıyla yapılmış kesitsel bir araştırmadır. Araştırma, Mart 2010 ve Eylül 2010 tarihleri arasında yürütülmüştür. Araştırmanın örneklemini, Park Sağlık Ocağı Bölgesi'nden orantılı tabakalama yöntemi ile seçilen 400 kadın oluşturmuştur. Veriler, araştırmacı tarafından geliştirilen ve kadınların sosyodemografik ve doğurganlık özelliklerini ve yöntem kullanımı ile ilgili etmenleri içeren 47 sorudan oluşan bir anket formu aracılığıyla, araştırmacı tarafından kadınlarla yüz yüze görüşülerek toplanmıştır. Veriler, SPSS (Statistical Package for Social Sciences) sürüm 15.0 paket programı aracılığıyla bilgisayara girilmiş ve istatistiksel analizlerde ki-kare testi, Kruskal Wallis varyans analizi ve tek yönlü varyans analizi kullanılmıştır. Yanılma olasılığı 0,05 olarak kabul edilmiştir. Araştırma grubundaki kadınların % 80'i halen yöntem kullanmakta olup, % 58,8'i modern, % 21,2'si geleneksel yöntem kullanmaktadır. Halen yöntem kullanan kadınların % 73,4'ü modern, % 26,6'sı geleneksel yöntem kullanmaktadır. Yöntem kullanan kadınlar 1. sırada RİA, ikinci sırada geri çekme ve üçüncü sırada kondom kullanmaktadır. Halen yöntem kullanan kadınların % 17,5'i danışmanlık almıştır. Konu ile ilgili benzer bir çalışmanın yapıldığı yıldan itibaren geçen son 15 yılda, PSO bölgesinde yaşayan 15-49 yaş grubu evli kadınların ilk evlenme, ilk gebelik ve ilk doğum yaşı ileri yaşlara kaymıştır. Modern yöntem kullanan kadın sayısı artmasına rağmen, doğurganlık da artmıştır. Yöntem bırakma oranı da oldukça yüksek olup, kadınlar en fazla oranda hap ve RİA yöntemini bırakmaktadır. Yöntem kullanmaya yeni başlayan kadınlar, geri çekme ve kondom yöntemini tercih etmektedir. Bütün bu faktörler bir arada değerlendirildiğinde, modern yöntem kullanımının artmış olmasına rağmen doğurganlığın artmasının, gebeliği önleyici yöntem kullanımında Birinci Basamak sağlık kurumlarına başvuru gerektirmeyen yöntemlerin tercih edildiği ve yöntem kullanan kadınların ancak % 17,5'inin danışmanlık hizmeti almasının rolü olduğunu düşündürmektedir. Bunun yanı sıra kadınların % 61,7'si yöntemlere karşıdır ve yöntem kullanım durumu değerlendirildiğinde kadınların % 20'den fazlası yöntem kullanırken gebe kalmış olup, en fazla oranda geri çekme yöntemini kullanırken gebe kalmıştır. Yaş arttıkça, isteyerek düşük sayısı da artmış ve en fazla oranda 2. gebelikte isteyerek düşük yapılmıştır. Yöntem kullanım durumu, öğrenim düzeyi, meslek ve KBYG'e göre değişmemiş, ancak yaşa göre değişmiştir. Benzer şekilde, modern/geleneksel yöntem kullanımı da öğrenim düzeyi, meslek ve KBYG'e göre değişmemiş; ancak yaşa göre değişmiştir. Özetle bu çalışmada; gerek modern gerekse geleneksel yöntem kullanımı ve genel olarak yöntem kullanımının gebelik, canlı doğum sayısına göre değişmiş olması yöntem kullanım durumunun yaşla birlikte değişen doğurganlık özelliklerine göre değiştiği ve danışmanlık hizmetinin alınmasının da önemli olduğu belirlenmiştir.AbstractThis survey which is a cross-sectional in design has been planned to indicate the contraceptive use and related factors among 15-49 aged married women in Park Health Centre Region, Ankara The study has been conducted between March and September 2010. The sample of the study has consisted of 400 married 15-49 aged women who have been selected by proportional stratified sampling method from Park Health Centre Region. The data have been collected by face to-face interview by the surveyor by a questionnaire including 47 questions regarding socio-demographic and fertility characteristics of the women and related factors influencing contraceptive use. The data have been analysed by SPSS (Statistical Package for Social Sciences) version 15.0, and in statistical analyses chi-square test, Kruskal Wallis variance analysis, and one-way analysis of variance have been used. The results were accepted to be statistically significant at the level of p<0,05. Eighty per cent of the women in the study population were currently using a contraceptive method at the time of this survey, and 58,8% were using a modern and 21,2% were using a traditional method. Seventy-three point four per cent of women who were currently using a contraceptive method were using a modern one and 26,6% were using a traditional method. Intrauterine device ranked the first, withdrawal ranked the second, and condom ranked the third among the contraceptive methods used. Despite a rise in use of modern contraceptive methods, the increase in fertility could be explained by the increase in the use of methods which were not needed to be applied at the first level health centre, and the decrease in receiving counselling health services such as 17.5%. The age at first marriage, first pregnancy, and giving the first live birth had moved one year forward in the last 15 years since a similar survey had been conducted in the same region. Although the use of modern contraceptive method had increased, the fertility had also increased. The ratio of discontinuation for the methods had been high, and combined with oral contraceptives and intrauterine device, were the most discontinued methods. The women, who had begun to use any contraceptive method, had preferred withdrawal and condom. So, according to these results, despite the increased use of modern methods, the increase in fertility indicated that when using a contraceptive method, the ones which did not need to apply to any health institution for use, were preferred by married women of reproductive age, and only 17.5% of the women had counselling. Also, 61.7% of the women were against contraceptive method use, and almost 20% of the women became pregnant while using a contraceptive method particularly while using withdrawal. As the age of the women increased, the number of induced abortions also increased, especially during the second pregnancy. The use of contraceptive method had not changed according to level of woman?s education, job, and yearly per capita income, but to age of the woman. Similarly, the use of modern/traditional method use did not also changed, but to age. Consequently, in this study, the use of modern/traditional and overall contraceptive method use had changed according to the number of pregnancies, and live births changing with the age of the women, and counselling had been revealed to be important in contraceptive use.
