Fotoferez işleminin immün sitokin profilinin çıkarılması

İlk kez kutanöz T hücreli lenfoma (KTHL) tedavisinde kullanılan fotoferez; otoimmün hastalıklar, solid organ nakillerinde doku reddinin önlenmesi ve GVHH için de kullanılmaktadır. Fotoferez işleminin etki mekanizması tam olarak bilinmemektedir. Çalışma için KTHL'li 6 hastadan, iki fotoferez seansının öncesi ve sonrasında kan alınarak eş zamanlı RT-PCR ile; IL-1, IL-2, IL-4, IL-5, IL-7, IL-15, GM-CSF VE TNF-? gen ifade düzeylerine bakıldı. Bulgularımıza göre; IL-1, IL-4, IL-5, IL-6, IL-7, GM-CSF ve TNF-? gen ifade düzeylerinde azalma, IL-15 gen ifade düzeyinde ise artış görüldü. IL-2 düzeyindeki artış, örneğin alındığı ilk işlem öncesi ile son işlem sonu arasında değerlendirildiğinde daha anlamlı bulundu. Elde ettiğimiz veriler fotoferez işleminin anti tümoral etki oluşturduğunu, fotoferez ile habis T hücre üretiminin durdurulduğunu ve KTHL'li hastalarda Th-1/ Th-2 oranını tersine çevirerek immün cevapta önemli bir etken olduğunu bununla birlikte tedavinin başarılı olabilmesinde, tedavi sürecinin ve hastanın bağışıklık sisteminin tedaviye verdiği cevabın önemli olduğunu düşündürdü. Sitokin düzeylerindeki bu değişikliklerin fotoferezin etkinliğini değerlendirmede bir belirleyici olup olmadığını söylemek için daha fazla vaka serilerinde değerlendirilmesi gerekmektedir.AbstractPhotopheresis was first used as a treatment of cutaneous T-cell lymphoma (CTCL). It is now also administered to treat autoimmune diseases, solid organ transplant rejection, and graft versus host disease. The mechanisms of action of this procedure is still unclear. Six patients with CTCL were diagnosed in this study. Blood samples were obtained before and after two photopheresis therapy and analysed IL-1, IL-2, IL-4, IL-5, IL-7, IL-15, GM-CSF, TNF-? gene expression levels by RT-PCR. According to our findings, we observed a decrease in IL-1, IL-4, IL-5, IL-6, IL-7, GM-CSF and TNF-? gene expression levels and also there has been an increase in IL-15. The increase in IL-2, when evaluated between the time prior to first process of the sample taken and the time just after the last process, was found more significant. Obtained results led us to think that the photopheresis process had an antitumoral effect and that it stopped the production of malign T cell. In immune response it played a effective active part in patients with CTCL by reversing the Th-1 and Th-2 proportion. However, for the treatment to be successful, the process of treatment and the response given by the immune system of the patient is important. Before making a conclusion about the value of cytokine changes in evaluating the effectiveness of photopheresis it could be evaluated on more patients.
