Girişimsel kardiyoloji incelemelerinde hasta dozlarının farklı yöntemlerle ölçümü

Kardiyoloji incelemeleri doz ölçümü yönünden son derece karmaşıktır. Kullanılanprojeksiyon, diğer bir deyişle hastanın ışınlanan bölgesi sürekli değişir; ayrıca hastayatağının ve görüntü güçlendiricinin hareketleri nedeniyle ışınlama geometrisinindeğişimi önemli diğer bir faktördür. Işınlama sürekli (floroskopik) veya birbirini takipeden statik görüntüler şeklinde elde edilir. Bu ışınlamalarda sistemin kVp, mA veışınlama süresi hasta kalınlığına bağlı olarak sürekli değişir. Bu çalışmada amaç, kardiyolojik incelemeler sırasında hastanın maruz kaldığı dozu vebu incelemelerde hasta dozunun belirlenmesi için en etkin yöntemin hangisi olduğunusaptamaktır. Hasta dozu ölçümlerinde farklı teknikler kullanılmıştır. Deterministiketkiler dikkate alındığında cilt dozunun ölçümü gerekir ve bunun için TLD(Termolüminesans Dozimetre) ve Gafkromik film detektörleri kullanılmıştır. Cilt dozuveya DAP (Doz Alan Çarpımı) ölçümleri kullanılarak hesaplanan etkin dozdanyararlanarak Stokastik riskler hesaplanmıştır. İncelemeler 5 farklı sistem ve toplam 325 hasta kullanılarak gerçekleştirilmiştir.İncelemelerde 10 projeksiyon kullanılmış olup hastanın cilt dozu, M4KDK sistemininküçük iyon odası, TLD ve gafkromik film kullanılarak ölçülmüştür. Hastanın her birbölgesinin etkin dozu, GD ve DAP ölçüm değerlerinin, Monte Carlo dönüşümfaktörlerinin kullanıldığı PCXMC programına girilerek elde edilmiştir. Elde edilensonuçlar literatür değerleri ile karşılaştırılmıştır. Abstract Cardiological examinations are extremely complicated considering dose measurement. Usedprojection, in other words the exposed area of patient, changes constantly; moreover, thegeometrical change in exposure due to the movements of patient-bed and image intensifier isanother important factor. Exposure is obtained as continous (floroscopic) or respective staticimages. In these exposures, kVp, mA and exposure-time of the system changes constinouslyaccording to patient thickness. The aim of this study is to determine the exposed dose of patinent during the cardiologicalexaminations and to find out the most efficient way of determining the patient-dose in theseexaminations. Various techniques have been used in patient-dose mearurements. When thedeterministic effects are taken into consideration, the measurement of skin-dose is necessary,and to do this, TLD (Thermoluminecance Dosemeter), and Gafcromic film detectors have beenused. Stochastic risks have been calculated using the effective-dose calculations obtained byskin-dose and DAP (Dose Area Product) measurements. Examinations have been carried out using 5 different systems and total of 325 patients. In theseexaminations 10 projections have been used and the patient skin-dose has been measured usingsmall ion chamber of M4KDK system, TLD and Gafcromic film. The effective dose of eacharea of the patient has been calculated by entering ED and DAP measurement values to thePCXMC program in which the Monte Carlo conversion factors are used. Obtained results havebeen compared with the ones in the literature.
