Canlı donör karaciğer transplantasyonu yapılan hastalarda biliyoenterik ve uç uca safra yolu anastomozlarının komplikasyonlarının karsılastırılması

Transplantasyon Ünitesi'nde canlı donör karaciğer transplantasyonu yapılan hastalarda uç uca safra yolu anastomozu ve duktojejunostomi anastomozunun nakil sonrası gelisen komplikasyonlarla arasında iliski olup olmadığını belirlemektir. Materyal ve Metod: Kliniğimizde canlı donör karaciğer transplantasyonu yapılan hastaların verileri retrospektif olarak kaydedilmis ve sonuçlar istatistiksel yöntemle karsılastırılmıstır. Hastaların yas ve cinsiyet dağılımları, Child ve MELD skorları, greft volümleri ve greft volümü/alıcı vücut ağırlığı oranları, preoperatif semptomları ve tanıları retrospektif olarak incelenmistir. Ameliyat sonrası gelisen komplikasyonlar; safra kaçağı, safra yolu tıkanıklığı, kolanjit, koledokolitiazis, biloma, intraabdominal apse, intraabdominal kanama, hepatik arter trombozu, hepatik ven trombozu, portal ven trombozu, rejeksiyon, reoperasyon ve mortalite seklinde değerlendirilmis ve bu komplikasyonlar sonrası hastalara yapılan balon ve stend girisimleri veya tekrar operasyon tedavileri kaydedilmistir. Bunların Roux-en-Y duktojejunostomi veya uç uca safra yolu anastomozu ile iliskisinin varlığı arastırılmıstır. Bulgular: 66 hastada duktojejunostomi yapılan grupta intraabdominal apse gelisimi istatistiksel olarak daha yüksek bulundu. (p:0.026) Diğer değerlendirilen komplikasyonlar ile anastomoz tipleri arasında anlamlı iliski bulunmadı. Sağ lob canlı donör karaciğer transplantasyonu yapılan hastalarda uç-uca anastomozda biliyer komplikasyonlardan safra kaçağında duktojejunostomi grubuna göre istatistiksel olarak anlamlı yükseklik saptanmıstır. (p:0.028) Sol lob canlı donör karaciğer transplantasyonu yapılan hastalarda uç-uca anastomozda biliyer komplikasyon yokken, enterik grupta biliyer komplikasyon olmus, ancak anastomoz tipleri arasında istatistiksel olarak fark saptanmamıstır. Anastomozdaki safra yolu sayısı 1, 2 ve 3 olan duktojejunostomi ve uç-uca safra yolu anastomozu yapılan hastalarda komplikasyonlar ile biliyer anastomoz tipi arasında istatistiksel olarak anlamlı fark saptanmamıstır. Hepatik arter trombozu, hepatik ven trombozu ve portal ven trombozu gelisen hasta grubunda safra yolu anastomozları ile komplikasyonlar arasında istatistiksel olarak anlamlı iliski saptanmamıstır. Sonuç: Canlı donör karaciğer transplantasyonlarında duktojejunostomi anastomozu grubunda intraabdominal apse gelisimi, sağ lob canlı donör karaciğer transplantasyonlarında uç-uca safra yolu anastomoz grubunda safra kaçağı istatistiksel olarak anlamlı yüksek bulunmustur. Karsılastırılan diğer komplikasyonlar ile safra yolu anastomoz tipleri arasında istatistiksel olarak anlamlı fark saptanmamıstır.AbstractThe aim of this study is to determine whether there is a correlation between the post transplantation complications and the type of bile duct anatomosis. Material and Method: Clinical data of living donor liver transplantations performed in Ankara University was collected retrospectively and the results were compared statistically. Age, sex, Child and MELD scores, graft volumes and graft volume/recipient body weight rates, preoprative symptoms and diagnosises were compared retrospectively. Postoperative complications taken under consideration were bile leakage, bile duct obstruction, cholangitis, choledocolithiasis, biloma, intraabdominal abscess formation, intraabdominal bleeding, hepatic artery thrombosis, hepatic vein thrombosis, portal vein thrombosis and rejection. The relation of those compliations and type of anastomosis whether Roux-en-Y ductojejunostomy or duct-to-duct bile duct anastomosis were compared. Findings: In the group that ductojejenostomy was performed rate of intraabdominal abscess development was significantly higher. (p:0.026) There were no significant correlation with the anastomosis type and the other reamining complications. Ductto- duct anatomosis group had significantly higher rates of bile leakage (p:0.028). In the evaluation of patients who underwent living donor left lobe liver transplantation bile leak was seen only in the bilioenteric anastomosis group but the statistical difference was not significant. Number of the ducts that were anastomosed had no impact on bile leak. There were no correlation between type of anatomosis and occurance of thrombosis of hepatic artery, hepatic vein and portal vein either. Result: Intraabdominal abscess formation was higher in ductojejenostomy group whereas bile leakage was more common in duct to duct anastomosis group. All other complications compared did not yield any statistical significant difference.
