Laparaskopik kolesistektomilerde postoperati ağrı kontrolünde deksametazon kullanımının analjezik tüketimine etkisi

Laparaskopik Kolesistektomilerde Postoperati Ağrı Kontrolünde Deksametazon Kullanımının Analjezik Tüketimine EtkisiBu çalışmada elektif laparaskopik kolesistektomi operasyonu planlanan hastalarda preoperatif dönemde intravenöz olarak uygulanan deksametazonun postoperatif ağrı kontrolündeki etkinliğini değerlendirmeyi amaçladık.Çalışmaya 18-70 yaş arası, ASA 1-2 olan, elektif laparaskopik kolesistektomi operasyonu planlanan toplam 60 hasta kabul edildi. Hastalar kapalı zarf usulüne göre 2 gruba ayrıldı.Hastalar ameliyathaneye alındıktan sonra; EKG, noninvaziv kan basıncıve periferik oksimetri ile monitorize edildiler. Anestezi indüksiyonu başlamadan önce Grup 1 `deki hastalara preoperatif olarak 8 mg deksametazon, Grup 2 deki hastalara da 2 ml %0.9 `luk NaCl intravenöz olarak verildi. Heriki gruba preempitif analjezik amaçlı 75 mg i.m diklofenak sodyum yapıldı. Hastaların intraoperatif dönemde kalp hızı, kan basıncı oksijen saturasyonları kaydedildi. Hastaların postoperatif dönemde 24 saat boyunca ağrıları görsel anolog skala (VAS) ile değerlendirildi. VAS>40 olan hastalara i.m 50 mg petidin uygulandı. Hastaların ek analjezik ihtiyaçları, bulantı ve kusma durumları ve yöntem memnuniyeti sorgulanarak kaydedildi.Çalışmamızda hastaların demografik verileri, VAS düzeyi, analjezik tüketimi ve hasta memnuniyeti açısından gruplar arasında anlamlı fark bulunmadı. Bulantı ve kusma durumu açısından ise gruplar arasında istatiksel olarak anlamlı fark bulundu (P<0.05).Çalışmamızda laparaskopik kolesistektomi planlanan hastalara preoperatif dönemde intravenöz olarak uygulanan deksametazonun postoperatif dönemde hastaların bulantı ve kusmasını azaltmasına rağmen postoperatif ağrı kontrolünde etkili olmadığı sonucuna varıldı. AbstractThe Effect Of Dexamethosone Usage For Postoperative Pain Management In Laparoscopic cholecystectomy Cases To Analgesic Drug ConsumptionThe aim of this study is to evaluate the effectivity aof intravenous dexamethasone application in the preoperative period for postoperative pain management in patients who are undergoing elective laparoscopic surgery.To this study 60 patients, aging between 18-70, with ASA scores of 1-2 undergoing elective laparoscopic cholecystectomy were included. The patients were divided in to two groups using the closed envelope method.ECG, non invasive blood pressure and periferic oxymetry monitorisation was applied to the patients after entering the OR. Before the induction of anesthesia while the patients of group 1 received 8 mg of dexamethasone, the patients of group 2 received 2 ml %0.9 NaCl solution intravenously during the preoperative period. 75 mg diclofenac sodium i.m. was applied to both of the two groups for preemptive analgesia. The patients heart rate, blood pressure and oxygen saturation values were recorded during the perioperative period. In the postoperative period the patients were evaluated for pain for 24 hours via visual analogue scale (VAS). To the patients with VAS scores over 40 (VAS>40). 50 mg i.m. Pethidine was applied. The need of analgesic, the cases with nausea and vomiting and whether the patients were content with this method or not were also evaluated and recorded.In this study, the difference between the two groups for demographic values, VAS scores, the analgesic consumption and patient satisfaction were not found significant. On the other hand there was a statistically significant difference between the two groups for nausea and vomiting (P<0,05).In this study intravenous dexamethasone application in the preoperative period for postoperative pain management in patients who are undergoing elective laparoscopic surgery we concluded that dexamethasone does decrease nausea and vomiting in the postoperative period but has no effect on postoperative pain management
