dc.description.abstractPrematüre anemisinde doku oksijenlenmesinin bozulduğu durumlar dışında allojenik eritrosit süspansiyonu (ES) transfüzyonunun yararları tartışmalıdır. Özellikle çok düşük doğum ağırlıklı (ÇDDA) bebeklerin ES transfüzyonu gereksinimleri çok fazladır. Bu nedenlerle prematüre anemisini engelleyici yaklaşımlar gündeme gelmiştir. Bu yaklaşımların temeli bu bebeklerin yüksek Hb düzeyi ile doğmalarını sağlamaktır. Böylece transfüzyon ihtiyaçlarının azaltılması veya engellemesi planlanmaktadır. Preterm bebekler doğum sırasında kordon hızlıca klemplendiğinde çok az plasental transfüzyon aldıkları için daha hipovolemik doğabilmektedirler. Bu da doğum sonrası geçiş sürecine olumsuz etki etmektedir. Bu çalışmanın amacı, ÇDDA prematürelerde plasental transfüzyonu arttırıcı yöntemlerden biri olan umbilikal kordonun sıvazlanması (KS) yönteminin, doğum sonrası hemotolojik, hemodinamik parametreler ve ES gereksinimi üzerine etkilerini değerlendirmekti. Çalışma, tek merkezli, randomize kontrollü olarak gestasyon yaşı 32 hafta ve/veya daha küçük olan prematüre bebeklerde Nisan 2011 ile Şubat 2013 tarihleri arasında yapıldı. Bebekler doğumdan hemen önce randomize edilerek iki gruba ayrıldılar: Grup 1, kordon sıvazlama grubu ve Grup 2, kontrol grubu. Kordon sıvazlama (KS) grubuna, plasenta ayrılmadan önce, bebeği sezaryen doğumlarda anne hizasında, vajinal doğumlarda ise plasentadan 30 cm aşağıda tutarak, göbek kordonunun 25-30 cm?lik kısmı plasental uçtan bebeğe doğru, 3 kez kuvvetlice sıvazlama tekniği uygulandı. Gruplar yaşamın ilk günü hematolojik ve hemodinamik özellikleri ve çalışma süresince ES transfüzyon gereksinimi ve neonatal morbiditeler açılarından değerlendirildi. Her iki grupta 22?şer olmak üzere toplam 44 bebek çalışmaya dahil edildi. Grupların gebelik gebelik yaşı (sırası ile 28.4 ± 1.8 ve 28.0 ± 1.9 hafta) ve doğum ağırlığı (sırası ile 1103 ± 236 ve 1232 ± 291 g) ortalamaları, demografik ve prenatal özellikleri benzerdi. Gruplar arasında yaşamın 1. saatinde ve 1, 7, 14. günlerinde bakılan Hb değeri ortalamalarının Grup 1?de belirgin yüksek olduğu saptandı. Yaşamın ilk 1 saati içinde bakılan ortalama kan basıncı ortancaları Grup 1?de daha yüksek saptansa da [sırası ile 38 (25-49) mmHg ve 32 (24-44) mmHg] arasındaki farkın istatistiksel anlamlılığa ulaşmadığı görüldü.Çalışma süresince ES transfüzyon gereksinimi hiç olmayan olguların oranı Grup 1?de 2?ye göre iki kat fazla olmasına karşın (sırası ile %21.1, %10.5) aradaki bu fark istatistiksel anlamlılığa ulaşmadı. Olguların yaşamlarının ilk 35 günü toplam ES transfüzyon sayı ve hacimlerinin ortancaları ile flebotomi kayıplarının ortancaları gruplar arasında farklı değildi. Gestasyon yaşı ne olursa olsun flebotomi hacminin ES gereksinimini etkileyen önemli bir değişken olduğu saptandı. Sonuç olarak, ÇDDA?lı bebeklerde KS?nin erken dönem hematolojik, hemodinamik parametreler ve ES gereksinimi üzerine olumlu etkileri gösterilmiştir. Flebotomi kayıplarının önlenememesi bu olumlu etkilerin ortaya çıkmasını engellemiş olabilir.AbstractThe benefit of transfusing allogenic erythrocyte suspension (ES) for anemia of prematurity is a controversial issue except for disturbances of tissue oxygenation. The need for ES transfusion in very low-birth-weight (VLBW) premature infants is very high. Therefore, preventive approaches of premature anemia have been raised. The basis of these approaches is providing high Hgb levels for the infants. Immediate cord clamping during the delivery of premature infants results in hypovolemia due to insufficient placental transfusion, which in turn can cause disturbances in postnatal adaptation. The aim of this study was to evaluate the effects of umblical cord milking (UCM) on hematologic and hemodynamic parameters in postnatal period and the need for ES transfusion in VLBW. The study was designed as a single center, randomized, controlled trial, conducted from April 2011 to February 2013 in infants born at or before 32 weeks gestation. The infants were randomized to two groups: Group 1 (UCM) and Group 2 (control). In the milking group, the infants were positioned at the same level with the mother in cesarean delivery or 30 cm below the level of placenta in vaginal delivery. About 25-30 cm of the umblical cord was milked vigorously toward the neonate for three times. The groups were evaluated for hematologic and hemodynamic parameters on the first day of life, and the need for ES transfusion and neonatal morbidities during the study period. A total of 44 infants were included with 22 infants in each group. There were no significant diffirences in gestational age (28.4±1.8 and 28.0±1.9 weeks, respectively), birth weight (1103±236 and 1232±291 g, respectively), and demographic and prenatal features between the two groups. The levels of Hgb on the first hour and 1, 7, 14 days were significantly higher in group 1. Mean blood pressure at the first hour after birth and urine output in first day of life were higher in group 1, but the difference did not reach statistical significance. The percentage of infants without any need of ES transfusion was two times higher in the UCM group than the control group (21.1% and 10.5%, respectively), but the difference was statistically insignificant. There was no difference in the number and median volume of ES transfusions, and median volume of phlebotomy losses between the groups. Phlebotomy loss was found to be an important determinant for the need of ES, regardless of the gastational age. In conclusion, UCM in VLBW infants is associated with potentially beneficial effects on hematologic, hemodynamic parameters during the first day of life, and on the need for ES transfusion. The emergence of these beneficial effects may have prevented due to phlebotomy losses can not be eliminated.
