Karpal tunel sendromunda kortikosteroid fonoforezinin ağrı ve el fonksiyonları üzerine etkisi

Bu çalışma, karpal tünel sendromu (KTS)'unda steroid fonoforezinin el fonksiyonları ve klinik parametrelere olan etkisini araştırmak ve bu etkileri ultrason (US) uygulamaları ile karşılaştırmak amacı ile planlandı. Çalışmaya, ENG ile idiyopatik KTS tanısı konmuş 60 hasta alınmıştır. Hastaların 54'ü (%90) kadın, 5'i (%10) erkektir. Hastaların ortalama yaşı 51.9'dır. Hastalar fonoforez (n=31) ve US (n=28 ) iki gruba randomize edildi. Fonoforez grubuna 1,0 W/cm2 dozda ve iletici ajan olarak % 0,1 hidroksikortizon pomad kullanılarak, steroid fonoforezi tedavisi uygulandı. US grubuna 1,0 W/cm2 dozda ultrason tedavisi uygulandı. Her iki gruba da sıcak uygulama (HP) tedavisi verildi. Tedavi dört hafta süre ile verildı. Hastalar tedavinin başlangıcında, sonrasında ve üçüncü ayında ağrı ve uyuşukluk vizüel analog skala (VAS)ile , semptom şiddeti ve fonksiyonel kapasite Boston Skalası ile, el becerisi ?Nine Hole Peg Test? ile, parmak ve avuç kavrama testi Jamar Dinanmometrisi ile değerlendirildi. Çalışmanın sonunda, steroid fonoforezi grubunda tüm değişkenlerde istatistiksel olarak anlamlı düzelme görüldü. US grubunda da VAS, Boston skalası semptom şiddeti ve fonksiyonel kapasite değerlerinde istatistiksel olarak anlamlı düzelme mevcuttu. KTS tedavisinde steroid fonoforezi ultrason ile karşılaştırıldığında KTS'li hastalarda mevcut olan klinik bulgular üzerine olumlu etkileri olduğu ve bu etkilerinin daha uzun sürdüğü görülmüştürabstractThis study has been planned to research the impact of steroid phonophoresia in carpal tunnel syndrome (CTS) on the hand functions and clinical parameters and to compare such results with the ultrasound (US) applications. 60 patients with the diagnosis of ENG and idiopathic CTS has been included in this study. 54 of the patient (90%) are female and 5 of them are male (10%). Average age of the patients is 51.9. The patients are randomly divided into two groups as phonophoresia (n=31) and US (n=28). Steroid phonophoresia is applied with the phonophoresia group using 0,1 hydroxicortisone at the dose of 1,0 W/cm2 and as transmitting agent. US group is applied with ultrasound treatment at the dose of 1,0 W/cm2. Heat application (HP) treatment is given to both of the groups. The cure is maintained for four weeks. The patients are evaluated in the beginning of and after the treatment and on the third month in terms of Boston Scale (symptom severity and functional capacity) with the visual analog scale (VAS), hand skills with `Nine Hole Peg Test? and finger and palm grasping test with Jamar Dynamometry. At the end of the study, meaningful improvements are statistically observed on all the variables of the steroid phonophoresia group. There is meaningful improvement statistically in values of VAS, Boston scale symptom severity and functional capacity. When steroid phonophoresia group is compared with the ultrasound treatment in CTS treatment, it has been observed that it produces positive impacts on the clinical findings of the patients with CTS and these impacts last longer.
