Dikkat eksikliği hiperaktivite bozukluğu olan kişilerde dikkatin uzaysal asimetrisinin incelenmesi
Özet
Dikkat Eksikliği Hiperaktivite Bozukluğu (DEHB) çocukluk çağının en sık görülen nöropsikiyatrik bozukluklarından biridir. Kendini dikkat eksikliği, aşın hareketlilik ve dürtü kontrolsüzlüğü ile gösteren bu klinik tablo bireylerin sosyal yaşantılarını, okul ve iş başarılarını olumsuz etkilemesi nedeniyle önem taşımaktadır. DEHB'nun etiyoloj isine yönelik hipotezlerin çoğu genetik veya çevresel etkenlerle beyin işlevlerinde ortaya çıkan bozukluklar üzerinde durmaktadır. Ancak bu bozuklukta kritik olarak etkilenen özgün beyin yapılan ve mekanizmaları hala tam olarak açığa kavuşturulmamıştır. Bugüne kadar DEHB'ndaki temel sorunun doğasını karakterize etmek ve açıklamak için geliştirilen teorik yaklaşımlar başlıca iki grupta toplanabilmektedir. Bunların bir grubu, dikkat kavramı ve dikkat teorilerine dayanarak patogenezi açıklamaya çalışan teorileri içerir. İkinci grupta ise, yürütücü işlevler kavramına dayanarak geliştirilen teoriler toplanmaktadır. Günümüzde, her iki teorik yaklaşımın da ortak nöral yapılara dayanmaları nedeni ile bir çatı altında birleştirilebileceği iddia edilmektedir. Araştırmalardan elde edilen bulgular, DEHB'nun altında yatan temel sorunun, sağ hemisferin kortikal ve subkortikal yapıların oluşturduğu frontostriatal devrelerde olduğu görüşüne yol açmıştır. Bu çalışma iki kısımdan oluşmaktadır. İlk kısım Laboratuvarımızda geliştirilen uzaysal ihmal testlerini bir batarya altında toplayarak çocuk yaş grubunda test-yeniden test güvenirliğini değerlendirmeyi, ikinci kısmı ise DEHB'nda sağ ve sol hemisferlerin motor ve uzaysal dikkatle ilgili işlevlerini incelemeyi amaçlamaktadır. Uzaysal ihmal test bataryası herhangi bir nörolojik sorunu olmayan 7-13 yaş arasındaki çocuklara uygulanmış (n=18) ve test-yeniden test güvenirliği incelenmiştir. Bataryada bulunan Parmak Vuru Testi, Küp Yerleştirme Testi (KYT), Çizgi Bölme Testi (ÇBT) ve Dönme Yönü Tercihi Testi yaklaşık bir ay ara ile aynı çocuklara uygulanmıştır. Parmak Vuru Testi her iki elin ince motor başarısını değerlendirmekte ve motor lateralizasyon için kanıt sağlamaktadır. KYT uzaysal işleyen bellek testidir. Aynı zamanda bu testte deneklerin gösterdiği ihmal, sağ ve sol uzaysal alanda ayrı ayrı değerlendirilmektedir. Laboratuvarımızda bilgisayar aracılığıyla deneklere sunulan ÇBT, uzaysal ihmale en duyarlı testlerden biridir. Dönme Yönü Tercihi Testi'nde, deneklerin ses uyaranına yanıt olarak dönmeleri sırasında sağdan veya soldan dönme tercihi incelenmektedir. Dönme yönü tercihinin dopaminerjik asimetri ve sağ hemisferin dikkatle ilgili işlevlerinden etkilendiği düşünülmektedir. Çalışmanın ikinci kısmına, 7-14 yaşları arasında, özgül öğrenme güçlüğü eşlik etmeyen ve ilaç kullanmayan DEHB tanısı almış 16 çocuk ve bunlarla yaş, cinsiyet, el tercihi ve okul yaşı yönünden eşleştirilmiş 16 sağlıklı çocuk katıldı. Tüm çocukların zeka puanlarının 85 ve üzeri olması gözetildi. Uzaysal ihmali değerlendirmek için ÇBT ve KYT kullanıldı. Bu testler, kullanılan elin ve işlem yapılan uzaysal alanın etkisini araştıracak şekilde düzenlendi. Dopamin ve dikkat asimetrisinin dolaylı göstergesi olarak Dönme Yönü Tercihi Testi uygulandı. Ayrıca Parmak Vuru Testi kullanılarak her iki grubun sağ ve sol el ince motor performansları değerlendirildi. Gruplar eşleştirilmiş T-testi ve Wilcoxon Testleri ile karşılaştırıldı. Test-yeniden test çalışmasında tüm testler genellikle anlamlı ve yüksek bir korelasyon gösterdi. KYT'nde düşük korelasyon gösteren ihmal sayılan ve lateralizasyon indeksi çalışmanın ikinci kısmında değerlendirilmedi. Bulgular sunulan bataryanın ilköğretim çağı çocuklarında güvenilir olarak kullanılabileceğini düşündürdü. Ayrıca KYT ve ÇBT'ndeki performansı gösteren verileri arasında anlamlı ilişki bulundu. Dönme Yönü Tercihi Testi'nde ÇBT ve KYT ile anlamlı bir korelasyona rastlanmadı. DEHB ve kontrol gruplan arasında ince motor performans farklı bulunmazken, ÇBT ve KYT testlerinde DEHB grubunun performansı kontrol grubuna göre anlamlı olarak düşük bulundu (p<0.05-0.001). ÇBT'nde, kontrol grubu sol alanda sunulan çizgileri anlamlı olarak orta noktanın soluna doğru bölerken (sağ el ile p<.001, sol el ile p<.01), DEHB grubunda bütün koşullarda orta noktadan anlamlı bir sapma gözlenmedi. Dönme Yönü Tercihi Testi'nde her iki grup da soldan dönme eğilimindeydi. Kontrol grubunun soldan dönme yüzdesi, %50 soldan dönme ile karşılaştırıldığında farklılık anlamlı bulunurken (p<.01), DEHB grubunun soldan dönme yüzdesi anlamlı bulunmadı. The symptoms of ADHD are hyperactivity, inattention and impulsivity, and this clinical table affects the school and academic life of patient with ADHD. ADHD is a biopsychological disorder, raising critical questions concerning the relations between genetic and environmental factors, but the exact mechanisms and the effected brain regions have not been understood well. Current main attempts to explain the core pathology of ADHD have been defined in the following two theories: The first one depends on attention concept and related theories, whereas the second one is a theory that explains the pathogenesis, depending on the deficit of executive functions. Nowadays, it is possible to combine these two theoretical attempts, because both are related to the same neuroanatomical structures. The results obtained from many investigations suggest that the main underlying problem of the ADHD pathogenesis is associated with frontostriatal circuitry in the right hemisphere. This research consists of two parts: The first part aims to investigate the test- retest reliability of neurocognitive tests used in this research for childhood population. The second part aims to investigate the spatial attention and motor functions of the right and left hemispheres in children with ADHD. In the test-retest reliability study, the Line Bisection Test, the Cube Placing Test, the Finger Tapping Test and the Rotation Preference Test were applied four weeks after first application to 18 normal children between 7 and 13 ages. Finger Tapping Test is a fine motor performance task, and gives us an evidence for motor lateralisation. Cube Placing Test is a working memory task, and performances of subjects are separately evaluated in the left and right hemispatial areas. Computerized Line Bisection Test is one of the most sensitive tests for neglect. In Rotation Preference Test, rotational bias of subjects is evaluated, considering rotation of subject to voice in a square shaped room. This bias is thought to be affected by dopaminergic asymmetry between the hemispheres. In the second part of research, 16 children between 7-14 ages and having ADHD were individually matched up with normal children for age, sex, handedness and school years. IQs of all these children were 85 or higher. These two groups of children took the Line Bisection Test, the Cube Placing Test, the Finger Tapping Test and the Rotational Preference Test. The results for both groups were compared with paired T-test and Wilcoxon Signed Ranks Test. In general, almost all test-retest correlation coefficients were found to be high and significant. These results suggest that the tasks used in this research are reliable for children in primary school period. In addition to that, correlations between the data of Cube Placing Test and Line Bisection Test were significant, but data obtained from the Rotation preference Test was not correlated with that of the Line Bisection Test and the Cube Placing Test. In Finger Tapping Test, there were no significant differences between two groups. In the Line Bisection Test and the Cube Placing Test, performances of the children with ADHD were significantly lower than that of normal children (p<.05-.001). In Line Bisection Test, the group with ADHD did not show a significant deviation from true center in all conditions, while the control group deviated from true center of lines to the left, when the line was presented in the left hemispace (with right hand p<.001, with left hand p<.01). In Rotation Preference Test, the both groups showed left turning bias. However, in comparison to 50% left turning, the left bias of control group was found to be significant (p<.01), although the group with ADHD showed no significant bias. The results supported the view, which suggesting that underlying deficit of the pathogenesis of ADHD is not a motor deficit, but a deficit of the frontostriatal circuitry at the right hemisphere.
