Sfingozinfosforilkolinin koroziv özofagus darlıklarının engellenmesindeki yeri (Sıçanlarda deneysel çalışma)

Güçlü koroziv maddelerin, özellikle alkalilerin içilmesi akut perforasyon ve ölümlere neden olabilir. Akut dönemi atlatan hastalarda ise gelişen özofagus darlığı sorun olmaktadır. Güçlü asit ve alkali gibi koroziv maddelerin içilmesi ile özofagusta oluşan yanıklara koroziv özofagus yanığı (KÖY) ve bunların etkileri sonucu oluşan darlığa koroziv özofagus darlığı (KÖD) adı verilir. Bu çalışmada 26 adet erkek albino Wistar kullanıldı. Standart KÖY modeli için Gehanno yöntemi ile %12 NaOH kullanılarak özofagus yanığı oluşturuldu. Gruplar 10 adet denekten oluşmak üzere iki gruba ayrıldı. Yanık grubuna plasebo, tedavi grubuna ise 7 gün süre ile oral yolla 0,2 uM sfıngozilfosforilkolin kullanıldı. Sıçanlar 21 gün sonunda özofagogramları çekildikten sonra öldürüldü. Sıçanların yanmış olan özofagus parçaları alındı ve KÖY oluşmuş segmentlerin (distal) radyografi k çapları, hidroksiprolin düzeyleri ve histopatolojik değerlendirmeleri yapıldı. Radyolojik incelemede Yanık grubunun% 80' inde tedavi grubunun % 30 da darlık oluştu. Distal dar segmentlerin çaplarının ölçümlerinde yanık grubunda 1,03 + 0,72 mm, tedavi grubunda ise 1,33 + 0,74 mm bulundu, fark anlamlıydı (p=0,021). Histopatolojik değerlendirmede yanık grubundaki bazı deneklerde rejenere skuamöz epitelin, normal örnekler veya tedavi grubu ile karşılaştırıldığında fazlaca düzleşmiş olduğu ve papilliform özelliğini kaybettiği dikkati çekmektedir. Yanık grubunun muskularis mukozasında kollajen birikimine bağlı kas liflerinin çapında azalma ve incelme ile karakterize atrofık değişiklikler görülürken tedavi grubunda bu bulgular gözlenmedi. Muskularis mukozanın kalınlığı ve kollajen miktarı tedavi grubuna göre anlamlı biçimde artmıştı (p=0,000). Yanık grubunda tedavi grubuna göre tunika mukozada yaygın fıbrozis ile hasar gelişti, fark anlamlıydı (P=0,000). Yanık grubundaki hidroksiprolin miktarının (4,32 + 0,23) tedavi grubunda ise (3,01 + 0,13) idi ve fark anlamlıydı (p=0,002). Bu çalışmanın sonucunda SPC nin KÖY' de darlık gelişimini engellediği ve SPC nin proliferatif, anjiyogenetik ve yaranın yeniden şekillenmesi etkileriyle yaranın iyileşmesinde etkin bir ajan olduğu görüldü.AbstractIngestion of potent corrosive agents, especially alkaline solutions may lead to acute esophagial or stomach perforation and death. Strictures of the esophagus may follow the acute phase in such patients. An experimental study has been carried out to investigate the effects of sphingosylphosphorylcholine (SPC) on the healing process following caustic esophageal burns. 26 male Wistar albino rats were used. Standard caustic esophageal burns were produced by %12 NaOH ussing Gehanno's metod. Following the caustic burn rats were divided into two groups, a experimental burn (EB) and a treatment group of 10 rats each. The EB group received placebo. The treatment group, received SPC 0.2 uM daily via orally for 7 days. Esophagography was performed and burned esophagial segments were excised on the 21st day of the experiment. The diameters of the burned segments were measured from esophagograms. Histopathological evaluation and analyses of tissue hydroxyproline levels were performed and compared on tissue samples. Although varying degrees of strictures developed in 80% of the EB group, a mild degree of stricture developed only in 30 % of treatment group. The mean diameter of the burned segment was significantly decreased in EB group (1,03 ± 0,72 mm) when compared to treatment group (1,33 ± 0,74 mm), (p<0,005). While epithelial regeneration was incomplete and obvious collagen deposition was present in submucosa, muscularis mucosa and muscular layer in EB group, epithelial regeneration was complete and collagen deposition was mild or absent in submucosa, muscularis mucosa and muscular layer in treatment group. The damage to muscularis mucosa and muscular layer was serious in all animals in the EB group whereas muscularis mucosa and muscularis layer was mildly injured in the treatment group. Thickness of the submucosa (p<0,005) and muscularis mucosa (p<0,005) was significantly increased in EB group. Histopathologic scores of submucosa (p<0,005), the muscularis mucosa (p<0,005), tunica muscularis (p<0,005) were significantly lower in treatmed group. Hydroxyproline contents of the burned esophagus were significantly lower in treatment group (3,0 ± 0,13 ug/mg), compared to EB group (4,32 ± 0,23 ug/mg) (p<0,005). The study suggest that administration SPC improves healing following caustic esophageal burns. Furthermore, SPC is effective in prevention of caustic esophageal strictures. These effects of SPC occur through its proliferative, angiogenetic and specifically remodelling effects on wound healing.
