Nonarteritik ön iskemik optik nöropatili hastalarda vasküler biyomarker olarak homosistein ve C-reaktif protein'in hastalık ile ilgili korelasyonunun incelenmesi

AMAÇ: Nonarteritik ön iskemik optik nöropati ( NAİON ) li ve nonarteritik iskemik optik nöropati için risk faktörleri içeren fakat NAİON si olmayan hastalarda homosistein ve C reaktif protein ( CRP ) nin plazma düzeylerinin hastalık gelişimindeki önemlerinin araştırılması ve çeşitli risk faktörleriyle aralarında bir koralasyonun olup olmadığının incelenmesi amaçlanmıştır. GEREÇ VE YÖNTEM: Ocak 2006-0cak 2009 tarihleri arasında Ankara Üniversitesi Göz Hastalıkları Anabilim dalında oftalmolojik muayene sonucu Nonarteritik Ön İskemik Optik Nöropati ( NAİON ) tanısı almış 25 hastanın toplam 27 gözü çalışmaya dahil edildi. NAİON olan hastalara rutin olarak tam oftalmolojik muayene yapıldı. Görme alanı, fundus fotoğrafı, tam kan, biyokimya, homosistein, CRP, B12 ve folik asit düzeylerine bakıldı. İki kontrol grubu oluşturuldu. Birinci kontrol grubunda hastalığın benzer risk faktörlerini içeren ancak NAİON olmayan 26 hastanın 52 gözü çalışmaya dahil edildi ( Risk faktörlü grubu ). İkinci kontrol grubuna benzer yaş grubunda olan ancak sistemik risk faktörlerini içermeyen 19 sağlıklı olgunun 38 gözü çalışmaya dahil edildi. BULGULAR: Nonarteritik ön iskemik optik nöropatili hasta grubunda risk faktörleri ile homosistein, B12 ve folik asit düzeyleri arasında anlamlı bir koralasyonun bulunmadığı görüldü. NAİON li hasta grubunda; hiperlipidemi, sigara kullanımı, cupless disk, diyabet-hipertansiyon-hiperlipidemi en sık risk faktörü olarak görüldü. C reaktif protein düzeylerinin yaş ile beraber lineer koralasyonda arttığı saptandı. Diğer kontrol gruplarına göre nonarteritik ön iskemik optik nöropatili olgularda cupless disk daha sık izlendi. Risk faktörlü hasta grubunda sigara kullanımı, diyabet-hipertansiyon-hiperlipidemi, hipertansiyon-hiperlipidemi, diyabet-hiperlipidemi daha sık olarak görüldü. Ancak bu grupta da diyabet, hipertansiyon, hiperlipidemi, sigara kullanımı ile homosistein, B12 ve folik asit düzeyleri arasında anlamlı bir koralasyon bulunmadı. Yine bu grupta da CRP düzeyi yaş ile birlikte lineer koralasyonda artış gösterdi. Risk faktörlü hasta grubunda CRP düzeyi NAİON li hasta grubuna göre daha yüksekti ancak bu durum istatistiksel olarak anlamlı değildi. Sağlıklı olgu grubunda plazma B12 düzeyi normal sınırlarda ancak diğer iki gruba göre anlamlı olarak daha düşük bulundu. SONUÇ: Çalışmamızda NAİON li ve NAİON si olmayan ancak küçük cup/disk oranı, diyabetes mellitus, hipertansiyon, sigara kullanımı, kolesterol ve trigliserit yüksekliği gibi risk faktörleri arasında homosistein ve CRP plazma düzeyleri ile bu risk faktörleri arasında anlamlı bir koralasyon tesbit edemememize rağmen özellikle cupless disklerde hastalık gelişiminde etkili olacağı kanaatine varılmıştır. AbstractObject: This study was aimed to investigate the significance of plasma levels of homocysteine and C-Reactive protein (CRP) on the development of Nonarteritic Anterior Ischeamic Optical Neuropathy ( NAION ) in patients with NAION and in patients without NAION but impliying risk factors of NAION, and to investigate the correlation between various risk factors and homocysteine and C-Reactive protein ( CRP). Material and Methods: The study was applied on 27 eyes of 25 patients diagnosed with Nonarteritic Anterior Ischeamic Optical Neuropathy ( NAION ) between January 2006 and January 2009. Complete ophthalmological examinations were regularly performed on patients with NAION . Visual field tests, complete blood count and biochemical analysis were performed, fundus images were taken and, levels of homocysteine, CRP andfolic asid were measured. Two control groups were employed . The first group composed of 52 eyes of 26 patients impliying similar risk factors of the disease but without NAION ( Risk Factor Group) . The second group were set up with 38 eyes of 19 healthy patients at the same age group who didn?t imply the systemic risk factors. Results: No significant correlation was found between the risk factors and the levels of homocysteine, B12 and folic asid in the patient group with Nonarteritic Anterior Ischeamic Optical Neuropathy . It was observed that hyperlipidemia, smoking, cupless disc and diabetes-hypertension-hyperlipidemia were leading risk factors in the patient group with NAION. It was found that level of C reactive protein increases with age in a linear correlation. Cupless disc was observed more often in cases with Nonarteritic Anterior Ischeamic Optical Neuropathy in conparion to the other control groups. Smoking, diabetes-hypertension-hyperlipidemia, hypertension-hyperlipidemia, diabetes hyperlipidemia were observed more often in patients without NAION but impliying risk factors of NAION. However no significant correlation was found between diabetes, hypertension, hyperlipidemia, smoking and the levels of homocysteine, B12 and folic asid in this group as well. CRP levels increased with age in a linear correlation for this group as well. CRP level of the group with risk factors was higher than that of the patient group with NAION, however it was not statistically significant. Levels of plasma B12 of the healthy group were within the regular limits, but significantly lower than that of the other two groups. Conclusion: No significant correlation was found between the risk factors- such as small cup/disc ratio, diabetes mellitus, hypertension, smoking, high cholesterol and high triglyceride ? and the plasma levels of homocysteine and CRP; however cupless discs together with these risk factors were determined to be effective in the development of the disease.
