Topikal prostaglandin analogu kullanımında gözlenen perioküler değişiklikler

Amaç: Topikal prostaglandin analogu kullanımında gözlenen perioküler değişikliklerin değerlendirilmesi ve ilaç kullanımının lakrimal drenaj sistemine etkisinin incelenmesi Gereç ve Yöntem: Toplam 69 kişinin 121 gözü Ankara Üniversitesi Tıp Fakültesi Göz Hastalıkları Anabilim Dalı'nda Mayıs 2009 ile Şubat 2011 tarihleri arasında prospektif olarak incelendi. Kırk üç hastanın 21'ine (35 göz, grup 1) bimatoprost tedavisi, 22'sine (34 göz, grup 2) latanoprost tedavisi verildi. Geriye kalan 26 katılımcı (52 göz, grup 3) sağlıklıydı ve kontrol grubunu oluşturmaktaydı. Tüm katılımcılar tedavi öncesinde ve tedavinin birinci, üçüncü, altıncı ve on ikinci ayında muayene edildi. Her kontrol muayenesinde palpebral fissür, kapak kıvrımı ve levator fonksiyonu bir cetvel aracılığıyla aynı araştırmacı tarafından ölçüldü. Üst kapak sulkus muayenesinde asimetri mevcutsa not edildi. Perioküler pigmentasyon, hipertrikozis, iriste pigmentasyon artışı, poliozis ve konjonktival hiperemi her kontrolde değerlendirildi. Yakın plan yüz fotoğrafları aynı kamera ile aynı araştırmacı tarafından çekildi. Tüm katılımcılara `Fitzpatrick Skin-Type Chart' anketi uygulandı. Soru-cevap yöntemiyle genetik yatkınlık, güneş ışığına reaksiyon ve bronzlaşma alışkanlıkları sorgulandı. İlaç kullanımının lakrimal drenaj sistemine etkilerini inceleme amaçlı hastalara her kontrol vizitinde lakrimal sistem lavajı yapıldı. Bulgular: Katılımcıların yaş ortalaması bimatoprost kullananlarda (grup 1) 63.9 ? 9.3 (46-78) yaş olarak, latanoprost kullananlarda (grup 2) 58.1 ? 11.6 (37-76) yaş olarak hesaplandı. Kontrol grubunda ise ortalama yaş 59.1 ? 10.6 (40-79) idi. Gruplar arasında istatistiksel olarak anlamlı fark bulunmadı (p>0.05). Çalışmaya dahil edilen toplam 69 kişinin 40'ı (%58) kadın, 29'u ise (%42) erkek idi. Gruplar arasında cinsiyet dağılımı açısından anlamlı fark bulunmadı (p>0.05). İzlem süresi sonunda gruplar arası palpebral fissür değerlerinin ve levator fonksiyonlarının zaman içindeki değişimi (grup-zaman etkileşimi) her üç grupta da anlamlı bulunmadı (p>0.05). Üst kapak kıvrımı açısından değerlendirildiğinde ise bimatoprost kullanan grupta sağlıklı kontrol grubuna göre istatistiksel olarak anlamlı yükselme tespit edilirken (p<0.05), latanoprost kullanan gözlerde izlem esnasında belirgin artış saptanmadı (p>0.05). İzlem süresi sonunda üst göz kapağı sulkusunda derinleşme, topikal bimatoprost kullanan grup 1'deki hastaların %34.3'ünde (35 gözün 12'si) (p<0.025), latanoprost kullanan grup 2'deki hastaların ise %2.9'unda (34 gözün 1'i) (p>0.025) tespit edildi. Perioküler pigmentasyon artışı grup 1'de %60 (35 gözün 21'i) (p<0.025), grup 2'de ise %26.5 (34 gözün 9'u) (p<0.025) oranında izlendi. Hipertrikozis sıklığı, grup 1'de %68.6 (35 gözün 24'ü) (p<0.025), grup 2'de ise %76.5 (34 gözün 26'sı) (p<0.025) olarak kaydedildi. İriste pigmentasyon artışı grup 1'de %25.7 (35 gözün 9'u) (p<0.025) oranında, grup 2'de ise %8.9 (34 gözün 3'ü) (p>0.025) oranında görüldü. Poliozis ise grup 1'de %17.1 (35 gözün 6'sı) (p<0.025), grup 2'de %5.9 (34 gözün 2'si) (p>0.025) oranında görüldü. Konjonktival hiperemi bimatoprost kullanan hastaların %51.4'ünde (35 gözün 18'i) (p<0.025), latanoprost kullananların ise %14.7'sinde (34 gözün 5'i) (p<0.025) saptandı. Toplam 69 kişinin 13'ünde (%18.8) tip II, 47'sinde (%68.1) tip III ve 9'unda (%13) tip IV cilt tipi tespit edildi. Perioküler pigmentasyon artışı ile cilt tipi arasındaki ilişkiyi incelendiğinde topikal bimatoprost kullanan hastalarda tip II ve tip III cilt tiplerinde, latanoprost kullananlarda ise tip III cilt tipinde tedavinin üçüncü ayından itibaren gelişen perioküler pigmentasyon artışı istatistiksel olarak anlamlı bulundu (p<0.008). İzlem süresi boyunca olguların hiçbirinin lakrimal drenaj sisteminde tıkanıklık izlenmedi. Sonuç: Topikal bimatoprost tedavisi, topikal latanoprost tedavisine oranla daha fazla perioküler değişikliğe sebep olur. Tek taraflı glokom olgularında bu ilaçlarla tedavi süresince ortaya çıkabilecek olası perioküler değişiklikler doktorlarca iyi bilinmeli ve prostaglandin analogları ile tedavi öncesi hastalar bu olası değişimler hakkında iyi bilgilendirilmelidir.AbstractPurpose: To evaluate the periocular changes due to topical prostaglandin analog use and to investigate the effects of these agents on lacrimal drainage system. Materials and Methods: One hundred twenty-one eyes of 69 participants were evaluated prospectively between May 2009 and February 2011 at Ankara University Faculty of Medicine, Deparment of Ophthalmology. Of 43 patients, 21 patients (35 eyes, group 1) were treated with bimatoprost therapy and 22 patients (34 eyes, group 2) were treated with latanoprost therapy. The remaining 26 participants (52 eyes, group 3) were healthy and accounted for the control group. All participants were examined before the prostaglandin therapy and then prospectively at one, three, six and twelve months of the therapy. On every examination palpebral fissure, upper eyelid crease and levator function were measured by a ruler by the same researcher. Upper eyelid sulcus asymmetry was noted whenever it was observed. Periocular hyperpigmentation, hypertrichosis, increased iris pigmentation, poliosis and conjunctival hyperemia were evaluated at every visit. All close-up fotographs were taken by the same researcher by the same camera. All participants had `Fitzpatrick Skin-Type Chart?. By this questionnaire genetic disposition, reaction to sun exposure and tanning habits were interrogated. To investigate the effects of the prostaglandin analog therapy on lacrimal drainage system, lacrimal system drainage irrigation was performed to all patients on every examination. Results: The mean age ? SD of group 1 was 63.9 ? 9.3 years (range 46-78), the mean age of group 2 was 58.1 ? 11.6 years (range 37-76). The mean age of control group was 59.1 ? 10.6 years (range 40-79). There was no significant difference between the groups (p>0.05). Of 69 participants, 40 (58%) were female, 29 (42%) were male. There was no significant difference between the groups (p>0.05). On the final examination, in terms of palpebral fissure and levator function, no significant change was identified during the follow-up in all groups (p>0.05). In terms of upper eyelid crease, in group 1 (bimatoprost use) a statistically significant increase was established according to control group (p<0.05), however in patients who use latanoprost there were no significant increase during the follow-up (p>0.05). At the end of the follow-up, the incidence of deepening upper eyelid sulcus was 34.3% (12 of 35 eyes) (p<0.025) in group 1 and 2.9% (1 of 34 eyes) (p>0.025) in group 2. The incidence of periocular hyperpigmentation was 60% (21 of 35 eyes) (p<0.025) in group 1 and 26.5% (9 of 34 eyes) (p<0.025) in group 2. The frequency of hypertrichosis was 68.6% (24 of 35 eyes) (p<0.025) in group 1, 76.5% (26 of 34 eyes) (p<0.025) in group 2. Increased iris pigmentation was seen 25.7% (9 of 35 eyes) (p<0.025) in group 1 and 8.9% (3 of 34 eyes) (p>0.025) in group 2. Poliosis was seen 17.1% (6 of 35 eyes) (p<0.025) of the patients in group 1, and 5.9% (2 of 34 eyes) (p>0.025) of the patients in group 2; conjunctival hyperemia was seen in 51.4% (18 of 35) (p<0.025) of the patients in group 1 and 14.7% (5 of 34 eyes) (p<0.025) of the patients in group 2. Of 69 participants, 13 had (18.8%) skin type II, 47 had (68.1%) has skin type III and 9 had (13%) skin type IV. We researched the relationship between the periocular pigmentation increase and skin type. In group 1, in patients with skin type II and III; in group 2, in patients with skin type III we found a statistically significant increase in pigmentation after the following three months of the therapy (p<0.008). During the follow-up no lacrimal drainage system obstruction was seen. Conclusion: Topical bimatoprost therapy cause more periocular changes than topical latanoprost therapy. In unilateral glaucoma patients, probable periocular changes should be well known by the doctors and patients should be well informed about these probable changes before prostaglandin analog therapy.
