Venöz yetmezliğin (Varis) cerrahi tedavisinde büyük safen venin ayak bileği-kasık arası çıkartılması (Stripping) ile diz-kasık arası çıkartılması yöntemlerinin hastaların semptomlar, yaşam kalitesi, nüks ve komplikasyonlar yönünden karşılaştırılması

Varisin cerrahi tedavisinde giderek artan bir sıklıkla uygulanmakta olan dizüstü stripping ya da diz üstü safen ven ablasyonu sonrası kalan bu bölümün klinik önemi yeterince bilinmemektedir Bu çalısmada venöz yetmezlik için BSV strippingi planlanan hastalarda venin ayak bileği-kasık arasının çıkartılması ile diz-kasık arası çıkartılması yöntemlerini prospektif randomize bir sekilde semptomlar, yasam kalitesi, nüks ve komplikasyonlar yönünden karsılastırmak amaçlanmıstır. Çalısmaya 26 hasta dahil edildi. Bunlardan 14 hasta erkek 12 hasta kadın idi. Hastalar 28 ila 61 yas aralığında olup yas ortalaması 40,53 idi. Hastalardan 19 unda hastalık tek bacakta iken 7 hasta da ise her iki bacakta da hastalık mevcuttu. Sonuç olarak 33 ekstremite prospektif randomize olarak tam büyük safen ven strippingi (19), ve diz üstü BSV strippingi (14) gruplarına dağıtıldı.Cerrahi islemi takiben, 3, 15, 30. günler ve 3, 6. aylarda değerlendirmeler yapıldı. Dizüstü ve tam safen ven stripping yöntemleri ilk 15 gün boyunca uygulanan numerik ağrı skoru (NAS), venöz sistem semptomları, yasam kalitesi, nüks ve komplikasyonlar yönünden karsılastırıldı. Tam stripping yapılan grupda sağ bacakta parestezi 3. gün, 1. ay ve 6.ayda istatistiksel olarak daha fazla bulundu. Diz üstü stripping yapılan grupda 6. ayda varis sikayeti tam stripping yapılan gruba göre istatistiksel olarak daha fazla saptandı. Tam stripping yapılan grupda Numerik ağrı skorunun (NAS) istatistiksel değerlendirmesi sonucunda sağ bacakta ikinci gün ağrı değerlendirilmesi anlamlı olarak daha fazla olduğu tesbit edildi. Diz üstü stripping yapılan grupta postoperatif 1. ayda SF-36 formu ile yapılan genel yasam kalitesi değerlendirmelerinin karsılastırılmasında genel sağlık yönünden avantajlı olduğu bulundu. Yine diz üstü stripping yapılan grup postoperatif 3. ayda SF-36 formu ile yapılan genel yasam kalitesi değerlendirmelerinin karsılastırılmasında mental sağlık ve genel sağlık yönünden istatistiksel olarak anlamlı sekilde avantajlı bulundu. Fakat postoperatif 6. ayda SF-36 formu ile yapılan genel yasam kalitesi değerlendirmelerinin karsılastırılmasında ilk aylarda olan farklılıklar ortadan kalktı ve bütün kategorilerde her iki gruba ait sonuçlar istatistiksel olarak benzer hale geldi. Diz üstü stripping grubu postoperatif 6. ayda yapılan venöz yetmezlik kaynaklı yasam kalitesi skoru(VEINES-QOL) yönünden yapılan degerlendirmede bacaklarda ağırlık hissi ve gece kramparı yönünden istatistiksel olarak anlamlı sekilde avantajlı bulundu. Yapılan varis tedavisinin etkinliğinin değerlendirilmesi amacı ile preoperatif ve postoperatif 6. aya ait SF-36 ve VEINES-QOL yasam kalitesi ölçeklerinin karsılastırıldı. Yapılan tedavi ile hemen tüm kategorilerde istatistiksel olarak anlamlı sekilde düzelme vardı. Yine yapılan varis tedavisinin etkinliğinin değerlendirilmesi amacı ile preoperatif ve postoperatif 6.aya ait VCSS verileri karsılastırıldı. Altı ay sonunda bütün parameterlerde istatistiksel olarak anlamlı sekilde belirgin düzelme vardı. Değerlenirilen diğer parametlerde ise istatistiksel olarak anlamlı fark saptanmadı. Her ne kadar bu çalısmanın takip süresi kısa olmasına rağmen, varisin cerrahi tedavisinde dizüstü stripping ilk asamada tercih edilmesi gereken bir tedavi seçeneği olduğu sonucuna ulasılmıstır.AbstractThe clinical importance of the remaining part of the saphenous vein after above knee stripping of above knee saphenous vein ablation is not clearly understood. The aim of the study is to compare ankle-groin great saphenous vein stripping and knee-groin saphenous vein stripping methods for symptoms, quality of life, recurrence and complications. 26 patients were included to the study. 14 were male and 12 were female. Age range was between 28 to 61 and mean was 40.53. 19 patients had unilateral disease whereas 7 had bilateral disease. A total of 33 extremities were randomized prospectively as complete GSV stripping (19) and above knee GSV stripping (14) groups. Following the surgical procedure, the patients were assessed at the end of 3rd, 15th, 30th days and 3rd and 6th months. The two groups were compared for purposes of venous system symptoms, quality of life, recurrence and complications with numerical pain score (NAS). Paresthesis in right leg was found to be significantly higher in complete stripping groups in 3rd day, 1st month and 6th month follow-ups. The complaints related to varicose veins were found to be significantly higher in above knee group in 6th month follow-up. In complete stripping group, statistical analysis of numerical pain score (NAS) revealed a higher perception of pain in postoperative 2nd day in right leg. Postoperative 1st month SF-36 quality of life assessment revealed an advantage in above knee stripping group. Again in the above knee group, postoperative 3rd month general quality of life assessment showed a significant advantage for the purposes of mental health and general health status. However, postoperative 6th month general quality of life assessment showed that all these differences had vanished and became statistically similar. Above knee stripping group had a statistically significant advantage regarding heaviness sensation in legs and night cramps assessed with quality of life score for venous insufficiency (VEINES-QOL). For the purpose of assessing the efficiency of treatment, preoperative and postoperative 6th month SF-36 and VEINES-QOL assessment were compared. There statistically significant improvement in nearly in all categories after treatment. Again, preopearative and postoperative 6th month data of VCSS were compared for the purpose of assessing the efficiency of treatment. There were statistically significant improvements in all parameters at the end of 6th month. Other parameters of the assessment had no significant difference. Although the follow up period of this study was short, it accomplished to conclude that above knee stripping should be the first choice of surgical treatment of varicose veins.
