Mide kanser radyoterapisinde iki boyutlu ve üç boyutlu tedavi planlarının karşılaştırılması

Amaç: Evre IB-IV, MO mide kanserinde postoperatif kemoradyoterapi standart tedavi olarak kabul edilmektedir. Çevredeki toleransı düşük kritik organlar nedeniyle mide kanserinde radyoterapi (RT) planlaması önemlidir.Bu çalışmada amaç mide kanseri radyoterapisinde iki boyutlu ve üç boyutlu tedavi planlarını dozimetrik açıdan karşılaştırmaktır. Böylece iki boyutlu (2B) planın alanlarının yeterliliği, 3 boyutlu (3B) konformal planın hem tedavi hedeflerini kapsaması hem de normal doku dozlarını düşürmesi açısından incelenmesi amaçlanmıştır.Gereç ve Yöntem: Evre II- IV mide adenokarsinomlu 36 hasta 3B konformal RT kullanılarak postoperatif adjuvan tedavi aldı. Her hasta için 2B konvansiyonel planlama ile ön-arka alanlar oluşturuldu. İki plan hedef hacmi kapsaması ve normal doku dozları açısından doz-hacim histogramları (DVH) kullanılarak karşılaştırıldı.Bulgular: Konformal tekniğin hedef hacmi daha iyi kapladığı. sol böbrek ve medulla spinalis dozlarının daha düşük olduğu bulundu; karaciğer dozları daha yüksek bulunmasına rağmen dozlar tolerans sınırları içerisindeydi.Sonuç: 3B konformal tedavi 2B'ye göre daha iyi doz dağılımı sağlar, sol böbrek ve medulla spinalis dozları daha düşüktür. Bu nedenle daha iyi tümör kontrolü ve daha az normal doku komplikasyonları ile sonuçlanacağı öngürülebilir.Abstract Background and purpose: Postoperatif chemoradiotherapy is accepted as a standard treatment for stage IB-IV, MO stomach cancer. Radiotherapy (RT) planning of stomach cancer is very important because of the low radiation tolerance of surrounding critical organs. The purpose of this study is to compare the dosimetric aspects of two-dimensional (2D) and three-dimensional (3D) treatment plans. In this way, evaluating the adequacy of two dimensional plannign fields on coverage of PTV and three-dimensional conformal plans for both covering PTV and reducing the normal tissue doses is aimed.Materials and methods: Thirty-six patients with stages II-IV adenocarcinoma of stomach were treated with adjuvant postoperative chemoradiotherapy using 3D conformal radiotherapy technique. For each patient, a second radiotherapy treatment plan was generated utilising AP-PA fields. The two techniques were then compared for target volume coverage and dose to normal tissues using dose volume histogram (DVH) analysis.Results: The conformal technique provides more adequate covarage of the target volume. Comparative DVHs for the left kidney and spinal cord demonstrate lower radiation doses using the conformal technique, and although the liver doses are higher, they are still below the tolerance doses.Conclusion: 3D conformal radiotherapy produces superior dose distributions and reduced radiation doses to left kidney and spinal cord compared to AP-PA techniques. For this reason it can be predicted that 3-dimensional conformal treatment will result in better tumor control and less normal tissue complications.
